25 | An Informal Office Meeting was conducted today with Monica Duran, Licensee. In attendance were Claretta Yates, Licensing Program Manager, and Kendal Zirbes, Licensing Program Analyst. The purpose of the informal conference was to discuss the serious type "A" deficiencies that the licensee was cited for on 12/08/21. Claretta Yates, LPM, discussed the following with Monica Duncan, the licensee.
On 12/08/2021, the licensee was cited three types “A “ citations: Section 102146.5(f) Staffing Ratio and Capacity. The licensee had 17 children in care, 5 of which were infants. The licensee was also cited for type "A" deficiencies Section 102416.5(d)(2) for having four infants in care when there was a total of more than 14 children., 102425(i) Infant Safe Sleep.
On 12/17/21, the Licensee was also cited type “B” for Section 102417(g)(10) Operation of a Family Child Care Home for A baby walker shall not be allowed on the premises of a family childcare home in accordance with Health and Safety Code Section 1596.846(b) and (c) On December 8, 2021, LPA observed two children utilizing baby jumpers in the family childcare home. These items are prohibited per Section 1500.86 of Part 1500 of Title 16 of the Code of Federal Regulations.
On 12/17/21, the Licensee was cited for one type “A” citation Section 102416.5(d)(2) Staffing Ratio and Capacity. Civil penalties assessed for repeat violation.
On 02/25/22, the Licensee was assessed Daily Civil Penalty for repeat violation Section 102416.5(d)(2) Staffing Ratio and Capacity, which were also discussed during this meeting.
