25 | On this date and time, Licensing Program Analysts (LPAs) Laura Mejorado and Susan Brewer arrived at the facility to conduct an inspection regarding a separate matter. LPA’s arrived and met with Marissa Duran, later in the inspection Director Alisa Holtegarrd arrived. LPA’s toured the facility, took census, and the following was discussed:
While touring the facility LPA's observed two preschool children using the bathrooms unattended. The door to the bathrooms were closed when the children were inside the bathrooms, each child entering the bathroom by themselves, while the teacher stayed outside in the classroom. Staff interviews revealed children are encouraged to close the bathroom door to teach them privacy. Children interviews revealed children use the bathroom alone, without assistance, and behind a closed door.
Therefore, based on LPA's observations the Facility was found to be in violation of the following Title 22 Regulation:
101229 Responsibility for Providing Care and Supervision
(a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time...

A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Exit interview conducted and report was reviewed with Director Alisa Holtegarrd. A copy of this report must be made available to the public, at the facility site, for 3 years.
