A case management inspection is being conducted in response to the receipt of an Unusual Incident Report (UIR) from the facility. Licensing Program Analysts (LPAs) Blanca Ruiz and Rachel Zeron met with the Director, Melissa Speakman to follow up on the UIR submitted to the department. Facility was toured and census was taken. Parent(s)/Legal guardian(s) of infant have been informed of the incident. Child#1 is still attending the facility.
Per Staff at the facility it was explained that on 05/09/2024 at approximately 11:30 a.m.Staff#5 at the facility reported that Child#1 in the Discovery Preschool Classroom was vomiting while class was transitioning from lunch to nap time. There were 18 children in care under the supervision of Staff#staff#5. When Staff # 5 noticed that a rash on Child#1's face started developed, child's parent was contacted immediately to report the first and second incident and pictured were provided for parent to assess the situation. Child#1 has severe allergies to dairy and egg and facility has taken precautions to prevent any incident from occurring while in care. It was also added that child's allergies are also trigger by touching surfaces and/or items in contact with these products.
Child was taken to urgent care and he was released the next day. Child#1 returned the facility the following week. It is unknown if the child consume and/or was expose to any foods or items at home or in the center the day in question.
During this inspection, LPAs verified food items in the menu, confidential allergy list in classroom/ kitchen to be posted for staff to be alerted of specific child(ren') needs. Child #1 has medication at the facility in case that an emergency occurred. Medication was verified to be in substantial compliance .
Per facility Director, Staff had a discussion regarding the incident to prevent irregularities with children food while in care. Based on information gathered, the facility acted appropriately, and no violations have been identified during this inspection.
An exit interview conducted, and report was reviewed with licensee, Melissa Speakman. Appeal rights were discussed, and A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. THIS REPORT MUST BE AVAILABLE TO THE PUBLIC, UPON THEIR REQUEST, FOR THREE YEARS.
