32 | It was alleged the facility is in poor condition. Ceiling tiles in the classrooms are broken, leaving areas of the ceiling exposed. Classrooms reportedly have a foul odor, a baby possum was found in one of the classrooms, rodent droppings are present throughout the facility, and there is no evidence of a pest management system in place.
On July 18, 2025, LPA conducted a tour of the entire facility, including the off-limits second floor. During the tour, LPA observed a first-floor classroom that is not currently in use and is being utilized for storage. In this classroom, LPA observed cobwebs, dust, debris, rugs, large toys, and stacks of paper piled on top of each other. While touring Classroom #108, LPA noted a foul odor but was unable to determine its source.
LPA further observed the second floor is also not in use and is similarly being used for storage. Unused items were stacked on top of each other. The area contained spider webs, dust, debris, rodent droppings, and yellow to brownish-red stains on ceiling tiles, floors, and rugs. Additionally, LPA observed empty buckets containing mold, mold accumulating on ceiling tiles, walls, and mold present in some of the toilet bowls in the upstairs restrooms.
LPA conducted interviews with pertinent parties. Pertinent parties stated staff reported on or about August 18, 2025, they were instructed to sweep up rat droppings found in a first-floor storage closet and additional droppings on the second floor. LPA reviewed a photo of a large possum that had reportedly left fecal smears inside the facility.
LPA inquired about pest management practices, but did not receive any documentation indicating that the facility engages in any form of pest control.
Based on LPAs own observations, documentation collected, and interviews conducted with pertinent parties, the department has determined the preponderance of evidence standard had been met, therefore the above allegations are found to be SUBSTANTIATED, according to the California Code of Regulations, Title 22, Division 12.

An exit interview was conducted, and a copy of this report and Notice of Site (NOS) Visit was provided to the facility representative, Kazi Akther. Upon request, this report must be made available to the public for three years.
