On this date and time, Licensing Program Analysts (LPAs) Laura Mejorado and Susan Brewer made an unannounced plan of corrections (POC) visit for citations issued during a previous inspection at the facility on 07/11/2023. LPAs met with Licensee Jocelyn Roces-Lopez, also present were 5 daycare children. LPAs toured the home inside and out, verified associations and discussed the following:

On 07/11/23 Licensee was cited for 102416.1(d) Personnel Records for S1. The POC was for Licensee to submit staff file for assistant present during the 7/11/23 inspection. The POC was due to the department by 7/14/23. During another inspection on 7/14/23 Licensee disclosed they still did not have the staff file for their assistant, the POC deadline was then extended to 7/21/23.

Today’s inspection is to obtain status on the POC which has not been corrected. During today’s inspection Licensee submitted a written statement stating they do not have a staff file for S1 who was present during LPAs inspection on 7/11/23. Licensee stated they have been unable to make contact with S1 and S1 has not returned since 7/11/23. Licensee stated they understand complete staff file's are required for all staff prior to working at the facility. Licensees written statement will be used as their POC for the 07/11/23 citation for 102416.1(d) Personnel Records.

A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
