On 2.6.25 at 10:27am Licensing Program Analyst (LPA) Kris DIaz conducted a visit at the facility to follow up on the Temporary Suspension Order (TSO) served on Salazar Family Childcare on 1.24.25 LPA was greeted by Licensee's daughter/assistant, Sarang Salazar. Licensee was not present, but arrived a few minutes later. At the time of the visit, LPA observed no children in care.

LPA observed the TSO notice posted at the main entrance of the facility visible to anyone arriving at the facility.

During the interviews conducted and videos viewed for complaint control #12-CC-20250108094151; it was concluded that two childcare children (C1, C2) wandered away from the facility.

The facility was cited Type A deficiencies. Please see Facility Evaluation Report dated 1.17.25 LIC 9099D for deficiencies.
