13 | Licensing Program Analyst (LPA) Aman Lama arrived at the center to initiate a complaint investigation regarding the above allegation. LPA met with the facility representative, Agatha Kolenda and stated the purpose of the visit. LPA toured the facility, took census, made observations, reviewed records and conducted interviews. LPA later met with the facility representative, Agatha Kolenda to deliver findings.
During the complaint investigation, it was disclosed that the director's last day at the center was 10-21-2025. The department was not informed of this information, nor was there a qualified staff present at the center. Therefore based on LPAs own observations, documentation collected, and interviews conducted with pertinent parties, the department has determined the preponderance of evidence standard had been met, therefore the above allegation is found to be SUBSTANTIATED, according to the California Code of Regulations, Title 22, Division 12. -See LIC809D for deficiencies cited.
An exit interview was conducted, and a copy of this report and Notice of Site (NOS) Visit was provided to the facility representative, Agatha Kolenda. Upon request, this report must be made available to the public for three years.
