On 06/28/2021 LPA Mabika met with Licensee, Lisa Burtner at Mc Donald Learning Center to conduct a Case Management visit to ensure the deficiencies cited during the last visit had been corrected.

A tour of the facility was conducted and LPA observed that:
  • The ceiling fan blades had been cleaned and a plan was put in place to periodically clean them.
  • The apple tree in the play center had been pruned off and no more had sharp edges.
  • The allergy lists had been covered for privacy
  • The school age calendar had been enhanced and new toys and games were purchased for use by this component.



Exit interview was conducted and a copy of this report was left with the Licensee, Lisa Burtner.
