Confidential interviews reported other staff and director was aware of child behavior and around the time the behavior occurs. No additional staff were assigned to assist with meeting C1 needs at the time of each incident; Therefore, the facility failed to provide care and supervision to meet the needs of C1. Based on the information obtained via interviews it is determined that current staff supervision and strategies allowed C1 to self-harm while in care. The allegation is valid because the preponderance of the evidence has been met, therefore, the above allegation is found to be Substantiated. California Code of Regulations, Title 22, Division 12, Chapter 1, regulation 101229 Responsibility for Providing Care and Supervision, is being cited on the attached LIC 9099D.
