25 | On August 23, 2023, at 02:30 p.m. Licensing Program Analyst (LPA) Kendal Zirbes met with Licensee Lisa Burtner. The purpose of the inspection was to conduct a health and safety check. LPA disclosed the purpose of the inspection to the Facility. Present during today’s inspection were 36 preschool age children, and one teacher and one aide..
At approximately 2:31 p.m. LPA observed two teachers supervising 36 children. Approximately 16 children were sitting at the table, four preschool age children were sleeping on cots in the main room and 13 children were laying on cots and walking around the nap room with staff 2. Staff 1 was observed preparing snack in the kitchen. Per conversation with the staff members the children woke up earlier than expected from their nap. Per conversation with staff members there were zero additional staff members on site to meet the teacher child ratio. LPA was informed that an additional staff member was on their way back to the Center.
A teacher-child ratio of one teacher supervising 24 napping children is permitted provided that the remaining teachers necessary to meet the overall ratio specified in Section 101216.3(a) are immediately available at the center. Based on LPA observation and interviews the Center failed provide the remaining teachers necessary to meet to the over all ratio for 36 children. Therefore a Type A citation for regulation 101230 (c) Activities is being issued as this is an immediate risk to the children in care. Refer to LIC 809D
LPA Zirbes informed licensee Lisa Burtner that this report dated 8/31/2023 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Zirbes informed the licensee to provide a copy of this licensing report dated 8/31/2023 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative Lisa Burtner.
