On September 20, 2022 at 2:31pm Licensing Program Analyst (LPA) Zirbes met with Licensee Lisa Burtner. The purpose of the inspection was conduct a plan of correction inspection for citations issued on September 1, 2022. Present during today’s inspection were six infants and two staff members.

During this inspection, LPA and License, reviewed the records. LPA observed the center is documenting the required 15 minutes checks. In addition, the facility has completed LIC 9227 for all infants under the age of 12 months. At the time of this inspection there was a staff member on site who had current CPR/First Aid training.

Based on LPAs observation and record review all citations issued on September 1, 2022 have been corrected.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Licensee, Lisa Burtner.
