25 | On October 30, 2024 at 03:45pm, Licensing Program Analyst (LPA) Kendal Zirbes conducted an unannounced Health and Safety inspection at the Center and met with facility representative Casie Gilham and Lilia Ross. LPA disclosed the purpose of the inspection and was granted entry into the facility by the facility representative Casie Gilham.
At 3:45 pm. LPA was on grounds for a different purpose and conducted a safety inspection. During the inspection, LPA observed staff 1 (S1) supervising seven awake infants and staff 2 (S2) supervising one sleeping infant in the napping room. The door between the napping and infant activity room was closed.
LPA discussed the observation with S1, who confirmed S1 was with seven children while S2 was with the one sleeping infant. LPA observed an aide in the kitchen and suggested the aide be brought into the classroom to met ratios. Per conversation with S1, the Licensee Lisa Burtner was planning on coming to the Center after completing a task at another facility. In addition, the other normally schedule staff member was out due to a medical appointment therefore extra staffing was not available.
Based on LPAs observation, S1 was supervising seven infants, therefore the Center was out of ratio. The Center was issued a type A citation for California Code of Regulations, Title 22, Division 12 Chapter 1, 101416.5 (b) Staff-Infant Ratio. Refer to the attached LIC 809D.
LPA Zirbes informed facility representative Lilia Ross Vollaire that this report dated October 30, 2024, documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Zirbes informed the facility representative to provide a copy of this licensing report dated October 30, 2024, that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
A notice of site visit was given and must remain posted for 30 days. An exit interview was conducted, and this report and appeal rights were reviewed with facility representative Lilia Ross Vollaire.
