On March 3, 2025, Licensing Program Analysts (LPA) Zirbes and Madox met with the facility representative Crystal Truax for the purpose of conducting an unannounced required Annual/Random inspection. LPAs toured and inspected the facility in accordance with the facility sketch. There were zero school age children on site during the inspection. The school age program is operated out of two classrooms (B4, B5). The center is located on Valley Community Chapel Campus. Therefore the Center utilizes the Valley Community Chapel's kitchen, office spaces and staff bathrooms located in the Social Hall. The center also operates a Preschool Program (364813646). The facility operates Monday through Friday 6:00am -6:00pm.
Indoor/Children's Area:
Child care center was observed to be clean, safe, sanitary and in good repair; all outdoor and indoor passageways, stair ways, incline, ramps, open porches and other areas of potential hazard are kept free of obstruction; floors of all rooms have a surface that is safe and clean. Cleaning compounds were stored in a key locked cabinet in B5. Per facility representative zero poisons are stored within the licensed spaces. LPA observed the furniture/equipment was in good condition. Classrooms were free of flies, other insects and rodents. The classrooms had tables and chairs to meet the children's needs. LPA observed the Center has a variety of art supplies, dramatic play equipment, library, board games, and additional materials for the children to engage in play. Children's belongings are kept in cubbies labeled with the children's names. Trash cans for solid waste have tight fitting lids. Drinking water is readily available indoors via personal water bottles provided by the authorized representatives and water coolers/cups. All materials and surfaces are toxic free, window screens are in good repair and free of debris. The carbon monoxide detector was observed in the classroom. The carbon monoxide detector was tested was found to be in working order. There was working telephone service within the classroom. There is an isolation area for children who become ill. The facility maintains a comfortable temperature at all times. LPA observed the centers electronic sign in/out sheets, which are completed daily.

Indoor/Children's Area continued: Smoking is prohibited on the premises. Teachers conduct daily inspection for illnesses. LPA did not observe prohibited childcare items during this inspection. Firearms/weapons are not allowed or stored on premises. LPA did not observe bodies of water on the premises. Facility representative stated there are zero bodies of water on the property.
Restrooms: The Center has a total of two children restrooms located in B4. Each restroom was equipped with one toilet and one sink. The restrooms provided the school age children with privacy. Restrooms were observed to be in safe and sanitary operating condition. Toilets and sinks are functioning properly and are age appropriate. Paper towels and soap were available to the children.
Meals: The Center provides three snacks a day during full day programs and two snacks when school is in session. The snacks are stored in each classroom. Menus were posted in the classrooms. Each family provides lunch for their child. The kitchen is shared with the Valley Community Chapel Campus
and is located in the off limits social hall.
Napping: The school age children do not nap at the Center however are provided with opportunities for quiet play when needed.
Outdoor
LPA inspected the outdoor play space for health, safety, cushioning material, good repair and age appropriateness. The School Age component has two outdoor play spaces. All outdoor spaces are enclosed by a chain link fence. One space was located near classroom B4 and B5. The area consists of dirt, cement and artificial grass. The area is utilized for large motor activities. There are three locked storage sheds in the area which held additional school supplies, janitorial supplies. LPA observed the addition play space is a large blacktop area. LPA observed a tether ball court, basketball hoops, sand box and dramatic play house. An approved waiver is on file allowing for the school age and preschool components to share playground space. Water is accessible to the children via drinking fountain and water coolers/cups and personal water bottles.
Due to time constraints LPA were unable to complete the entire annual inspection. LPA will return on a future date to complete the inspection.

Based on LPAs observations today, there were zero citations issued today. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative Crystal Truax.
