On January 15, 2025, Licensing Program Analyst (LPA), Calloway conducted an unannounced case management inspection to the above facility. LPA met with the Site Director who granted access. LPA toured the facility with the Director and observed thirty-two-day care children in active care and four- napping preschool day care children and six staff. LPA observed the doors to the classrooms were changed to half doors and are no longer identified by colors. Per Director, the classroom doors were changed after day care hours. LPA observed a child using the restroom with a night light and teacher assistance.

On November 15, 2024, Licensing Program Analyst made a visit to the facility and spoke to the Director, who stated there was a construction project going on for at least a month where the ceiling was being dropped down inside the day care center and the orange classroom was closed due to the renovation. LPA toured the facility and observed the day care center was dark on the main floor because the preschool children were napping. There were seven school- aged children in their area that were sleeping on the couches, sitting in the chairs, and sitting on the floor against the wall, and one child was sitting on the floor with their head in their knees they all were in the dark inside their area. There was a vent cover missing in the ceiling in the main day care area underneath where the preschool children were napping. The orange classroom had a visibly exposed ceiling, several unopened boxes of ceiling tile, a box labeled 25 ft length class insulated flexible duct was sitting on top of a table.

Inside the employee break room, there were ceiling tiles unboxed, and stacked behind the employee’s couch, the furniture was shifted around and there was light dust around the baseboards. The yellow room had storage supplies and items stacked up high and was not locked during the inspection a teacher was asked to lock it.

Per the Director, the renovation work and painting were done after the daycare hours and no children were present. However, any alterations to the existing building shall be reported to the licensing department and the need for a waiver will be assessed. The facility did not report the building alterations to Licensing and the school aged children were in a dark space with no other lighting options.

There are two Type B deficiency cited during this inspection. See 809D pages attached to this report.

Exit interview was conducted and a copy of this report was read, a Notice of Site Visit, and Appeal Rights were provided to Katrina Henderson, Director at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.

Per Director, for the school aged area we will get table lamps or turn the overhead lights on for that area and the restroom will have the overhead lights were the sinks are that turn on the lights inside the toilet area on at all times.
