On 02/25/2025, Licensing Program Analysts (LPAs) Ana Rodriguez and Carol Heath met with Director Jennette Honnold and Assistant Director Sandra Hardy to conduct an unannounced case management inspection. The purpose of the case management was to follow up on unusual incident report (UIR) received 02/10/2025. Incident occurred on 02/07/2025, Facility reported two children hurt each other On Friday 2/7/25 One child bit the other child. Both parents were notified of incident. Ice pack was applied.
Upon arrival, LPAs observed 18 childcare children and 3 teachers providing care and supervision.

During this inspection LPAs conducted interview with Director, Assistant Director, and other related parties. LPAs observed 1 childrens file and 4 staff files. In addition, LPAs completed a safety inspection of the facility grounds. In addition, during the inspection, LPAs obtained copies of documentation of incident reports, health checks, and facility roster.

Due to the need to gather additional information, the case management will require further investigation.

A notice of site visit was given and must remain posted for 30 days. Failure to keep these posted for the entire 30 days will result in an immediate $100 civil penalty for each.

(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.

(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.
