On the date and time listed above, Licensing Program Analyst (LPA) Aman Lama arrived at the facility to conduct a case management-Licensee initiated inspection. Upon arrival, LPA toured the facility and took census. LPA was later met with a substitute site supervisor, who was arriving from another site, Liz Martinez.

The reason for the inspection was to inspect the new outdoor play structure. No square footage was added to the area. During the inspection, LPA read off the "warning labels" on the outdoor play structure found on a pole to the left side of the slide. The play structure has been cleared to utilize as soon as the facility would like. There is soft cushion to absorb falls around the high-climbing areas.

See LIC809D for citation.

- The facility is reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

- The Duty Officer is available to answer questions Monday – Friday; 8:00am to 5:00pm at: 951-782-4200

LPA conducted the exit interview and reviewed report with substitute site director, Liz Martinez.

A notice of site visit was given, posted, and must remain so for 30 consecutive days.
