LPA reviewed the discipline policy. The policy states, the facility will provide support to authorized representatives of children with challenging behaviors by conducting meetings, provide additional resources, and if need be, evaluate the appropriateness of the school environment for the child.
From February 2024 to August of 2024, facility staff had 4 meetings with a child’s authorized representative due to the child’s challenging behaviors. During the meetings, facility staff suggested additional outside resources for the child; however, facility staff failed to evaluate the appropriateness of the school environment for the child after the child continued to display challenging behaviors and failed to provide a safe environment for individuals.
On 08/12/24, after the complaint investigation was initiated, the director stated an extra staff member was added to the classroom to help mitigate inappropriate interactions between children and to provide a safe environment for all individuals.
Based interviews conducted and facility policies reviewed, facility did not provide a safe environment which allowed inappropriate interactions between children and staff; therefore, the allegations are substantiated, meaning the preponderance of evidence has been met.
LPA informed director Tiffany Carey, that Type A citation must be reported to all authorized representatives/guardians of all children currently enrolled by the next business day, or the next day children are in care, and all newly enrolled children for the next 12 months from the date of citation. The signed Acknowledgement of Receipt LIC 9224, must be placed in child’s file for verification.
Exit interview conducted, report, appeal rights, acknowledgment of receipt and notice of site visit issued.
Notice of Site Visit must be posted for 30 days. |
