On August 13, 2024, Licensing Program Analyst (LPA) Calloway met with Licensee, who granted access. LPA and Licensee toured the home inside and outside for an Annual Random inspection. Residing in the home are the Licensee, and three minor children. The home is providing care and supervision for twelve to fourteen children. LPA observed five (5) day care children in active care and four (4) napping children with licensee and two facility representatives’ during inspection.
Physical Plant: This is a two story 5-bedroom, 3-bathroom home with kitchen, dining room, family room, living room, and garage. Childcare is provided: in Living room upon entry, Bedroom #1 on the left in the hallway. Children’s Bathroom: is in hallway on the right it was observed clean and in good repair with no hazards. There were soap and toilet paper available, sink and toilet were operable. Unused outlets were covered. Blind cords were not tied up high. Age-appropriate toys, furniture, and books were observed. Napping equipment were cots and playpens and enough for all children. LPA observed a changing table and high chairs. The home was inspected inside and out for safety, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents/cleaning compounds were kept inside a locked hallway closet medicine were inside the kitchen inaccessible on a high shelf to the right of the stove.

Kitchen: the hazardous items that can pose a danger to children and sharp knives were inside a cabinet to the right of the stove locked. Fire/earthquake drills both were complete 8/2/2024. Roster was incomplete with physician names and phone numbers and parent’s names. The required fire extinguisher (2A10BC) was full/green under the kitchen sink with a safety latch. Smoke detectors and carbon monoxide detectors were tested as operable. Fireplace is screened. Home has central AC and heat. Required postings were present on the wall. No landline phone, but cell phone. Stairs were gated. Fire alarm pull was not observed. LPA informed licensee an updated fire inspection is required. Per licensee, the surveillance cameras inside the daycare area are operable.

Off limit areas: Garage (key lock), Upstairs: Bedrooms: #2, #3, #4, #5, Bathrooms: #2, #3, laundry. Gated areas behind the play area inside the backyard.


Outside: The backyard is completely fenced. Pets: Two small dogs were observed inside the kitchen near the sliding glass inside a kennel. Per Licensee, the dogs are vaccinated and do not interact with the day care children. There were several Little Tykes toys, slide sets and other age-appropriate toys, bikes, and play equipment. No pool/spa or body of water on the premises. The AC unit was not covered. LPA recommended a mesh, breathable cover.
Others: Per Licensee, there are no weapons or firearms on the premises. LPA did not observe any in the home. Required mandated reporter training is current. CPR/First Aid (Exp: 10/24), and immunizations are current. First Aid kit was observed. LPA viewed staff and children’s files and reviewed all infants in care have LIC 9227 form in their files. LPA conducted a staff interview with the licensee. Transportation is provided. Incidental Medical Services (IMS) policy and Safe Sleep regulations were discussed. The following was discussed with the Licensee:
Maintain capacity and transparency per posted parent rights, Roster requirements (keep updated information always), Documentation for disaster drills (fire and earthquake).

Mandatory forms, signed, for the children’s files and provider’s files, updated Safe Sleep regulations. The role and responsibilities of being a mandated reporter were reviewed. Licensee reminded that supervision is always required to children in care. If food is brought in, it is properly labeled. Licensee will check food expiration dates periodically. Licensee was advised on how to access forms and regulations for Family Child Care online at www.ccld.ca.gov. Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care and temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day. If the day care is closed for the day, no kids show up, or Licensee is absent, Licensing must be notified. Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must always have the facility’s phone number; if the phone number is changed, licensing must be notified. Regulations prohibit the smoking of tobacco in a private residence that is licensed as a family childcare home and areas of the day care home where children are present (24/7 ban). State law prohibits baby walkers, bouncy seats, exersaucers, and any other items that fall into that category.

Type A deficiency: Type A deficiency shall be posted for 30 consecutive days along with the Notice of Site Visit Letter (printed out after every visit) and posted during hours of operation, as there is an immediate risk to the health, safety, or personal rights of children in care. Licensee shall provide a copy any Type A deficiency to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of the Type A report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224) must be placed in the child's file for verification of the Type A deficiency. Failure to do so will result in a civil penalty being assessed.

LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Per Title 22 Regulations, Division 12, Chapter 1, there were no deficiencies cited during this inspection. There was a Technical Violation and Technical Advisory issued. See the TV/TA pages attached to this report.

An exit interview was conducted, a copy of this report was read, and a Notice of Site Visit were provided to Elvia Wade, Licensee. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain the posting will result in $100 civil penalty.
