On 09/17/2025, the Facility Evaluation Report was amended with the purpose of dismissing the citation given on the same day of 05/15/2025.

On May 15, 2025, Licensing Program Analysts (LPA’S) Giovanni Cristales and Crystal Ali met with the facility teacher Annabelle Fox, for an unannounced annual inspection. A tour of the facility was conducted. Upon arrival, LPA’S observed 1 Infant classroom in use with a total of 6 Infants children. There were 2 infant teachers on the premises. The facility also has pre-school (364842462) and school age (364842463 - inactive) components. The center operates 6:30AM -5:30PM Monday through Friday. Incidental Medical Services (IMS) policy was discussed.

Indoor/Children's Area: Indoor activity space for infants is physically separate from space used by children in the preschool and school-age childcare center components. Infant center is equipped with appropriate furniture and equipment including 2 cribs, 12 cots, 1 changing table, 2 feeding chairs, and 2 swings. Furniture is maintained in good repair and safe condition (age appropriate). Toys are safe and LPA’S did not observe sharp points or edges, or small parts that could be swallowed. Per teacher, equipment, toys and furniture are sanitized daily. Window screens and blinds (no cords) in good repair. There is one hand washing sink within arm’s length of the changing table. There is another sink used only for bottle cleaning as needed. LPA’S observed no infant left without supervision (visual), nor left unattended. The facility is following the staff/infant ratio and is not exceeding the approved capacity for the facility.



The center was inspected for cleanliness, safety, sanitation, repairs, potentially hazardous areas (passageways), floors inaccessibility of cleaning compounds, poisons locked, furniture and equipment, and play equipment are age appropriate. Each infant has own cubby, trash cans have tight fitting lids, drinking water is available, no toxic materials observed, window screens in good repair.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPA’S observed the carbon monoxide, Fire Extinguisher (Service Date: 8/26/24), inside infants’ room on wall, inaccessible to infants and smoke detector in working condition. The center meets the requirements of the Fire Marshall (Last visit date: 2/2025). There is a working telephone on the premises, isolation area for ill infants in dining area, room temperature is comfortable, sign in/out log observed, no smoking, and no weapons on the premises, no baby walkers or other prohibited play equipment on the premises. LPA’S observed 1 bathroom for staff. LPA'S observed cameras in common areas.

Kitchen & Food Service: LPA’S observed safety gate with lock being accessible to children. The safety latch on saloon swinging door not effective. There is a fully equipped kitchen with refrigerator, stove, microwave, and sink. Refrigerator temperature in regulation. The kitchen was clean and there were no visible hazards observed. Knifes and sharp tools were stored in the top 4th cabinet to the right of the stove on middle shelf, inaccessible to infants. Silverware is cleaned, sanitized, and put away after each use. The facility provides morning snack, breakfast, lunch, and afternoon snacks. The menus are posted inside/outside the class. There is a confidential allergy list posted inside the cabinet. LPA’S observed an appropriate (non-expired) amount of food and snacks. LPA’S observed 1%, nonfat milk and whole milk for infants/toddlers transitioning and staff items were in a separate area. The chemicals and cleaning supplies, medicine and first aid kit are kept separate in the supply room (off limits) with a safety latch at top right of door, and away from the food.



Napping: LPA’S observed 2 cribs, and 12 cots in the infant room. There are no objects for which infants can use to climb out of a crib. No cushion around inside of crib. Each crib is used for one infant at a time, bedding changed daily or as needed, cribs and cots are arranged to allow staff to reach an infant without having to step over an infant. The changing table is padded with a washable plastic, in good repair, and is not located in the kitchen area. LPA’S observed cots in safe condition, sanitized after each use, cots spaced to have access to a safe walkway, bedding is not in contact with other bedding. Center provides blankets for children. LPA’S observed blankets in top cabinets. Per teacher, blankets are washed once a week (every Friday).

Infant Food Service: LPA’S observed the individual feeding plan for each infant, high-chairs and seating equipment in good repair. Food brought from home is labeled with infant name and date. One child has a food allergy. Food allergies are documented in the child file and posted in the kitchen cabinet to the left of kitchen sink.

Lead Water Testing: Per teacher, lead water testing was completed last year.


Transportation: Facility does not provide transportation.

Outdoor: Activity space is physically separate from the space used by children in the preschool and school-age center component with a white picket fence and in good condition. The outdoor space is maintained in a safe condition and is free of hazards. Center has a shaded area, play-ground free from hazards and debris, play structure has four swings (anchored) cushioning material, sandbox. Per teacher, playground is checked and inspected daily. Center is free of flies or other insects and rodents.

Health Related Services: Medications when used are inaccessible to children, child name and date on medication, parent written consent as required. Per teacher 1 infant is under medication due to teething.

Facility Records: LPA’S observed the Roster to be complete and current, staff has current CPR/First Aid expires: 3/28/2026, Fire and Earthquake Drills log last completed on 12/23/2024. Mandated Report Training were reviewed. Staff and infant files were reviewed and are in compliance. LPA’S observed safe sleep logs.

Posting Requirements: The following were observed posted as required: facility license, Personal Rights (LIC613A), Parent’s Rights Poster (PUB 394L), Emergency Disaster Plan, and Earthquake preparedness checklist.

Criminal Record Clearance - Child Care Centers


Facility representative was reminded that all adults 18 and over, including
employees and volunteers, except as specified in Health and Safety Code section 1596.871,
must obtain a criminal record clearance or exemption, or transfer their existing clearance or
exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00
minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a
maximum of 30 days per person will be assessed if this regulation is violated.

Lead Testing – Child Care Centers (CCC)
CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES:
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care
Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every
5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for
lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-
CCP). LPA verified that the lead testing was completed in accordance to the Written Directives
outlined in PIN 21-21.1-CCP.
