Licensing Program Analyst (LPA) Thompson-Miller met with the Director, Luz Marisol Valenzuela and conducted an Annual Random inspection. LPA toured and inspected the Infant program in accordance with the facility sketch. LPA observed six infants (five eating, one napping) and one Teacher, one Aide pon arrival. The facility consists of two classroom (divided by size of children). The center also operates an School Age Program (#364843657) and Preschool (#364843656)Program. The center operates 6:00am-6:30pm, Monday through Friday. Incidental Medical Services (IMS) policy was discussed.

Indoor/Children's Area: Indoor activity space for infants is physically separate from space used by children in the preschool and school-age child care center components. Infant center is equipped with appropriate furniture and equipment including cribs, cots, changing table and feeding chairs. Furniture is maintained in good repair and safe condition (age appropriate). Toys are safe and LPA did not observe sharp points or edges, or small parts that could be swallowed. Per Director equipment, toys and furniture is sanitized daily. There is one hand washing sink within arms length of the changing table. No infant is left without supervision (visual), nor left unattended. The facility is in compliance with the staff/infant ratio and is not exceeding the approved capacity for the facility.

Furniture and equipment were inspected for age appropriateness and good repair low chairs and feeding tables have broad based legs, chair trays lift and lock, low chairs are washable, changing table padding thickness in regulation and is within arm's length. Potty training is not done for this age group (toilet training begin at age 2 years). Drinking water is available inside the classroom (sippy cups, bottles). LPA observed staff with infants during lunch and nap time. Bedding is changed daily for the younger infants and older infants. The bedding is stored separately and properly. There is a Infant Daily Report and a Drop Off information (when was last time infant ate/sleep, last diaper change, how long slept, how much ate) sheet for parents to complete. Used diapers are put in trash container with lid (diaper genie). Each infant has container labeled (name) with personal items. Infant room is clean and safe. There are 2 evacuation cribs along with other cribs for the infants.

The center is clean, safe, sanitary and in good repair, there are no potential hazardous areas observed (passageways), floors are clean and safe, cleaning compounds are inaccessible, poisons locked, furniture and equipment in good condition, play equipment are age appropriate. Each infant has own cubby, trash cans have tight fitting lids, drinking water is available, no toxic materials observed, no fireplace in the room, window screens in good repair. LPA observed the carbon monoxide, Fire Extinguisher and smoke detector in working condition. There is a working telephone on the premises, isolation area for ill children at the office area, room temperature is comfortable, First Aid Kit observed, sign in/out observed, COVID-19 protocol observed (temperature check), no smoking on the premises, no baby walkers or other prohibited play equipment on the premises.

Children/Infant are inspected for illnesses (wellness policy) as they arrive. A review of medication policy indicated that prescription medication is administered only with parent's written permission (licensing medication form- LIC9221 - also used) and a doctor's note. The Director administers medication and documents the dosage, date and time onto a log. Medication can be brought and taken home by the parent daily. Medication is properly labeled and stored in its original container. There is a separate area for isolation and care of ill children at the entrance with the Director or Director office.

Napping: There are cots for infants that can climb out of a crib. Each crib is used for one infant at a time, bedding changed daily or as needed, cribs and cots are arranged to allow staff to reach an infant without having to step over an infant. The changing table is padded with a washable plastic, in good repair, and is not located in the kitchen area. LPA observed cots in safe condition, sanitize after each use, cots spaced to have access to a safe walkway, bedding is not in contact with other bedding.

Infant Needs and Service Plan: Teacher child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Infant file has Needs and Services Plan/Individual Sleep Plan. Staff were questioned to establish their familiarity of emergency reporting requirements, emergency disaster plans and other site operations. Personal rights of children were discussed. Children's records and staff records were reviewed.
LPA observed the Infant Needs and Service Plan in each infant file (updated quarterly).
Infant Food Service: LPA observed the individual feeding plan for each infant, low chairs and seating equipment in good repair. Food bought from home is labeled with infant name and date (currently no outside food brought inside the center, beside breast milk).

LPA discussed the safe sleep regulations with licensee [or facility representative] and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Outdoor: Outdoor play equipment was inspected for health, safety, good repair and age appropriateness. There is a dirt, sand, turf and concrete area for active play. The area was observed to be free of debris. There is an area for shade and rest (patio, trees). Drinking water is available (Staff have backpack which contains sippy cup and first aid material). Staff inspect outside play area prior to children exiting to the play area. Play area was inspected for hazards and inaccessibility to bodies of water. Outside area is completely fenced. Activity space is physically separate from the space used by children in the preschool and school-age center component. The outdoor space is maintained in a safe condition and is free of hazards, cushion material to absorb fall(padding). Center is free of flies or other insects and rodents.

Center has a shaded area, play ground free from hazards and debris, play structure has cushioning material, sandbox inspected daily, air condition unit is inaccessible.

Health Related Services: Medications when used are inaccessible to children, child name and date on medication, parent written consent as required.

Food Service: There is a kitchen area (door kept locked). Chemicals (locked) and food are kept in separate cabinets. Parent can bring breast milk (stored in the kitchen and labelled)--currently one parent bringing breast milk. The center provides breakfast, lunch and PM snack (baby food meals). Menus are posted in the lobby and kitchen. Allergy lists are posted in kitchen. LPA observed an appropriate amount of food and snacks for infants during the inspection.

Food prepared and served in a safe manner, food preparation is completed in the kitchen, cleaning compounds are kept in the supply room (off limits), refrigerator temperature in regulation, menu posted in kitchen and classroom, breakfast, snack and lunch are provided and as the infant feeding is necessary.

Sign in and out sheets were reviewed. LPA observed Licensing and Parent boards to be incompliance including the required posting (menu, daily schedule, community activities, licensing required documents). Fire/earthquake drills current. Roster current and maintained. First Aid Kit observed. CPR/First Aid expire 3/28/23. Note: The training must be taken through American Red Cross, American Heart Association, or through Emergency Medical Services (EMS) Authority approved trainers.

Staff and infant files were reviewed and documented. Medication – all medications are stored with child’s name and date. Sign in/out sheets are complete (Procare Application).

Staff Records: LPA reviewed files for Director Qualifications, immunization, licensing forms (LIC9108, Mandated Reporter, LIC508, LIC308).

Facility Records: LPA observed the Roster to be complete and current, staff has current CPR/First Aid expire 03/28/23, fire and earthquake drills are conducted monthly.

Posting Requirements: Center has required posting (waiver, license, PUB 393L, car seat law, emergency disaster plan, earthquake preparedness, lead poison).

Telephone service was verified (cordless and intercom system). Heating, lighting, and ventilation are adequate.

LPA reviewed with Director the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Any duly authorized officer, employee, or agent of the Department shall, upon presentation of proper identification, shall inspect the facility. The licensee/Applicant shall permit the Department to inspect the family child care home, and to privately interview children or staff, to determine compliance with or to prevent violations of family child care laws or regulations, also enter and inspect any place providing personal care, supervision and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation.
