On 12/17/2024, Licensing Program Analyst (LPA) Carol Heath and Ana Rodriguez conducted an unannounced Annual inspection at the facility and met with Director Alyse Sanders. LPA explained the purpose of the visit and was granted access to the facility. Upon arrival, LPAs observed 2 preschool classrooms for the preschool program, with 12 children and one preschool teacher present. According to the director, due to low enrollment, the center only used 1 preschool at this time. The facility operates from 6:00 AM to 6:30 PM, Monday through Friday. The center also includes an Infant Program (364843655) and a School-age Program (364843657). During the visit, Incidental Medical Services (IMS) were discussed.
Physical Plant: LPA thoroughly inspected the preschool classrooms, evaluating furniture and equipment for age appropriateness and condition. All items were in good repair and free from sharp or loose parts. The floors were clean and safe, and carbon monoxide detectors were present in each classroom. The daily activity schedule was observed, telephone service was verified, and heating, lighting, and ventilation were adequate. Each child has a labeled cubby for personal belongings, and drinking water is provided in classrooms via disposable cups and water jugs.
LPA also noted that all waste containers, including movable bins, had tight-fitting lids and were leakproof and rodent-proof. Disinfectants, cleaning solutions, poisons, and other hazardous items were securely stored outside children's reach. Classroom temperatures were confirmed to be no more than 20 degrees cooler than the outside temperature.

Fire and earthquake drills are conducted every six months, and the disaster drill log is posted in the office. All dangerous items were locked and inaccessible to children.
Napping: Sufficient cots were available for napping, with bedding stored separately and appropriately. Sheets and blankets are either washed daily or sent home for cleaning, and cots are sanitized daily. Each cot or mat had a covering sheet.
Health Services: LPA inspected the First Aid kit, which contained all required supplies, including a first aid manual, sterile dressings, bandages, adhesive tape, scissors, tweezers, a thermometer, and antiseptic. Children are screened for illnesses upon arrival, and wellness checks are conducted. Prescription medication is administered with parental consent, and a designated staff member logs all dosages. Medication is stored securely and labeled correctly, and once expired, it is returned to parents for disposal. The front office is the designated area for isolating ill children.
Bathrooms: LPA inspected the bathroom facilities and observed one clean bathroom (two toilets, three sinks). The toilets and sinks are functioning correctly and are age-appropriate. LPA observed that soap, toilet paper, and towels were readily available. The water temperature is appropriate; if hot water (temperature 105 degrees F and 120 degrees F) is available, hand washing fixtures used by children are not required to deliver hot water. One toilet and one sink are available for ill children, staff, and emergencies.
Outdoor Area: The outdoor play area was inspected, and the fences were at least five feet high. The facility has 1 playground area for the preschool children. The preschool children play in a gated area separated by a wall from the infant children's play area. The outside play area is completely fenced. Outdoor play equipment was inspected for health, safety, good repair, and age appropriateness. There is dirt, grass, and concrete area for active play. The area was observed to be free of debris, free from hazards, holes, broken items, debris, and dry grass; cushioning material underneath (sand, grass), free of hazardous items; six feet fence surrounding the outdoor area; air conditioning equipment, water heaters, fuse boxes inaccessible.

No bodies of water were observed. There is an area for shade and rest. Drinking water in a pitcher-Igloo and Dixie cups are available, as well as a water fountain.
Transportation Arrangements: According to the Director, transportation is provided only for school-age children. Sign-in and sign-out sheets confirmed the proper teacher-to-child ratio was maintained.
Staff Records/Children's Records: LPA reviewed staff and children's records. At least one staff member present had current CPR and First Aid training, and all staff members had completed Mandated Reporter training. Required vaccinations were current, and fingerprinting was confirmed for all staff. LPA also reviewed 5 children's files, verifying signatures and documentation were complete.
Staffing Ratio and Capacity: Teacher-to-child ratios were observed, with staff names recorded. Care and supervision were assessed to ensure children's needs were appropriately met.
Food Service: The facility has a clean, fully equipped kitchen with a refrigerator, freezer, stove, and microwave oven, which is off-limits to children. The center provides breakfast, lunch, and a PM snack, with allergy lists prominently posted. The food preparation area was clean, and chemicals were stored separately. Menus are posted a week in advance for parents to review. The menu and parent board are posted in the kitchen. The allergy list is posted in the kitchen. LPA observed an appropriate amount of food and snacks. The chemicals are kept separate from the food (kitchen cabinet). LPA did not observe cook's MMR and Dtap information. The TB test is Neg on 9/6/2022.
Parent Board: LPA observed the facility license, menus, parental rights, child passenger restraint system poster, Emergency Disaster Plan, Earthquake preparedness, and daily activity schedule displayed on the parent board.

The following general information was discussed during this inspection:
Immunization Requirements: §1596.7995 (a)(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a daycare center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.
*All Licensing reports are available for review online and are considered public information. Summary: Assembly Bill 2621 added Section 1596.819 to the Health and Safety Code, requiring the Department to post certain licensing information for CCCs and FCCHs on its public internet website.
Fingerprint clearances and transfers: Before working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review shall obtain a clearance or criminal record exemption. If a fingerprint clearance has been obtained through the Department, Licensee may request a transfer of a criminal record clearance from one state-licensed facility to another using form LIC 9184

**Senate Bill AB 633 - Child Care Facilities: Parent Notification Requirements
Summary: This bill amends Health and Safety Code (HSC) sections 1596.859, 1596.8595, 1596.8895, and 1597.05 to improve the transparency of licensing records and to ensure that parents/guardians using a licensed childcare facility are aware of situations that present the greatest danger to children
**Center Director advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation (call within 24 hours and submit a written report within 7 days) and on the form LIC624. The Center reminded to review updates/regulations as well as quarterly updates on the Department website. The "Notification of Parent's Rights" poster must be posted in an area of the center accessible to parents. The information regarding new legislation with regard to exemptions and parents' rights was also discussed.

Criminal Record Clearance - Child Care Centers Licensee [or facility representative] was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
Lead Testing – Child Care Centers (CCC) CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.
