On Friday, April 25, 2025, Licensing Program Analysts (LPAs) Mayra Rivera and Giovanni Cristales conducted an unannounced complaint inspection regarding staff do not follow p rocedures to mitigate the spread of illness. Upon arrival, LPA Rivera met with director Alesha Stenner who granted access and guided LPAs Rivera and Cristales on a tour of the facility. LPAs entered classrooms toddlers yellow, Blue 3’s, Red 4’s and 5’s and green 2’s. LPAs observed a total of 31 children and staff #1, staff #2, staff #3, staff #4, and staff #5 present providing care and supervision.

During the course of this investigation, LPA Rivera and LPA Cristales conducted confidential interviews, observed children and staff with no illness and reviewed documentation. Based on the statements made during the interviews, LPAs observation and documentation staff are following procedures to mitigate the spread of illness. During morning drop off, children’s temperatures are taken, handwashing when children are done playing and before their meals. The classrooms surfaces are being sanitized with Clorox wipes and

Signs of sickness or signs of fever, the temperature in taken to confirm if child has a fever, and if the temperature is above 100, the child is isolated. Isolation takes place in the director’s office. Staff notifies director when a child shows symptoms of illness and director contacts parents to notify and to pick up the child. The child remains isolated until the parent arrives for pick up. The facility also follows the 24-hour sent home illness policy. They may not return to school for a minimum of 24 hours following the end of any of the following fever over 100 with no medication, diarrhea, vomiting, pink eye, sore throat, heavy nasal discharge, evidence of no head lice.

This agency has investigated the complaint alleging staff do not follow procedures to mitigate the spread of illness. At this time, it is determined that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore at this time the above allegation is unsubstantiated. No deficiency given at this time.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit made by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.



Exit interview was conducted with director Alesha Stenner. The director was provided a copy of the appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these forms.
