Licensing Program Analyst (LPA), Aman Lama arrived at the facility to conduct an annual inspection. LPA was met with site director, Stephanie Armendariz. Later, LPA was met with licensee, Melody Houl. LPA toured the facility inside and outside, and the following was observed and/or noted:
The following documents are requested to be updated, if any changes have been made, and submit to the Riverside Regional Office (RRO) within 30 days of this report:
1. LIC 500 Personnel Report
2. LIC 610 Emergency & Disaster Plan
3. Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule (only if changes have been made)
4. LIC 309 Administrative Organization
5. LIC 308 Designation of Administrative Responsibility

The following items have been posted and are updated where necessary:

· The facility is operating within the limits as stated on the license.

·LPA observed bodies of water in the outdoor play area of the facility. At least 3 out of 5 wading pools still had water in them, and they were all still laying flat on the ground. All wading pools or similar product must be emptied immediately after use and stored in an upright position-SEE LIC809D
· Parents have the option to bring their children’s own water bottles, but there is an option for drinking water from water jugs and disposable cups, containing uncontaminated drinking water, as needed.
· Hazards are stored where they were accessible to children. These include: Disinfectants, cleaning solutions and other items that are dangerous to the health and safety of children in care-SEE LIC809D
· Poisons and toxins are to be kept locked and inaccessible to children, per Title 22 Regulations.
· All floors were observed clean and safe and shall remain so at all times.
· Restrooms were observed to be sanitary and in operating condition.
· Playgrounds are enclosed by appropriate fences.
· Outdoor activity areas are in good condition and supplied with age/size appropriate equipment.
· The areas around or under high climbing equipment, swings, slides, and similar equipment were cushioned with material that absorbs a fall.
· Food preparation area is clean, free of litter, rubbish and free of rodents and other vermin.
· Food is stored appropriately and protected from contamination.
· Menus were posted with date included and were placed in a visible location of children’s authorized representatives. Menus shall be kept on file for 30 days, and made available upon request.
· All storage containers for solid waste, including moveable bins have tight-fitting covers that were observed to be on and in good repair.
· Sign in/Sign out record was reviewed and meets regulation requirements.
· There is at least one Closing and one opening staff present with a current CPR/First Aid on file.
· A review of children’s records were found to be complete during this inspection.
· Disaster drills are to be conducted every six months – last drill was conducted on 06/05/24.
· The facility is informed of their reporting requirements and is provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov
· A review of staff records indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
· The facility can submit transfer forms to associate or disassociate someone from their facility at: Associations_Disassociations862@dss.ca.gov

·A review of staff records indicates that staff that were present meet minimum qualifications for the position for which they were hired. -SEE LIC809D

- This facility is not currently providing Incidental Medical Services (IMS). LPA discussed storage of medications and required documents needed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226.

- The facility was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA conducted the exit interview and reviewed report with licensee, Melody Houl.

LPA Aman Lama informed the site director, Stephanie Armendariz to provide a copy of this licensing report dated 07/09/2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given, posted, and must remain so for 30 consecutive days.

Based on observation during the facility tour, LPA found multiple "Lysol" disinfectant sprays in the 4-4.5 year old classroom. These items were not behind a lock or a latch and were kept in a cabinet which was easily accessible for the age group of children in the classroom. Additionally, there is a classroom which is not in use by children, but children go into to wash their hands. LPA found screws and a screw driver in reach of children. This all poses an immediate health, safety or personal rights risk to persons in care.

Based on observation during the tour of the outdoor area, LPA observed 5 wading pools with at least 3 wading pools that still had a few inches of water left in them. Site director stated Tuesdays are their "water play" days and she typically goes out afterwards to empty the wading pools and also put them away. Due to all 3 classrooms having exit doors that lead directly to this area of the facility, this poses an immediate health, safety or personal rights risk to persons in care. Children were having lunch during the time LPA found the water in the wading pools.

Site director was able to immediately dump the water out of the wading pools and store them away. Additionally, facility site director agrees to hold a meeting with all staff regarding Section 101238.5, Wading Pools of Title 22 Regulations and submit proof of agenda to licensing by POC due date.

Based on observation LPA observed multiple trash cans in all 3 rooms without lids on them. One trash can had a regular sized "Coppertone" sun screen tube thrown into it, and another trash can had food that was bitten/not eaten in it. All 3 classrooms were observed without lids on their trash cans, with food in them and/or other items. This poses/posed a potential health, safety or personal rights risk to persons in care.
