25 | On 12/09/2024 Licensing Program Analyst (LPA) Justin Giese made an unannounced visit to the facility to follow-up on the submission of an Unusual Incident Report (UIR). The UIR outlined a potential incident which occurred on 11/19/2024 pertaining to Personal Rights. LPA met with Site Director, Desiree Escobar.
According to UIR, on 11/19/2024 it was reported a pertinent individual alleged facility staff had taken a pinky ring from a child in care. It was stated law enforcement were called and conducted interviews/incident report pertaining to this matter.
On 12/09/2024, LPA met with Site Director and recorded the following information. According to Director, on 11/19/2024, law enforcement was contacted after alleged incident had occurred. Statements from staff were recorded by law enforcement and staff conducted a sweep of the facility in an attempt to locate the child's ring. Facility Director stated staff searched the playground and classroom thoroughly and were unable to locate the child's ring. On 11/21/2024, staff located the child's ring within the facility classroom, It was located in a basket of magnetic toys. It was reported the child either removed the ring while they were playing with the toys or it got attached to the toys during play and was removed. Director stated the ring was returned to the child's authorized representative.
The facility fulfilled their reporting requirements and reported the incident when it has occurred. The child's ring was eventually located and returned. There appear to be no violations of Title 22 or deficiencies at this time.
An exit interview was conducted and this report was reviewed with Site Director. A notice of site visit was issued and was posted in a prominent location in the facility.
Notice of site visit must remain posted for the next 30 days. |
