On 06/29/21 at 09:52am Licensing Program Analysts (LPAs) Justin Giese and Giselle Carbulllido made an unannounced visit to the Facility for the purpose of conducting a complaint investigation. LPAs met with Director, Shannon Garcia to discuss above allegations and deliver complaint investigation findings.

The following was alleged: Facility is operating beyond the conditions and limitations specified on the license.LPAs investigated the above allegation and gathered the following information:

While touring the facility LPAs observed 6 school aged children in attendance comingled in the same classroom with preschool children. Interviews with the Director and staff confirm the facility is providing care for School aged children. The facility currently does not have a license to provide care for school aged children.

Based on LPAs observations and interviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.

Please see attached LIC9099D.

The following was alleged: Unqualified staff supervising children. LPAs investigated the above allegation and gathered the following information:

Director and staff interviews confirm that on 06/09/2021 unqualified staff were left to supervise a classroom of children that were conducting their distance learning. Staff's own admission confirm they were left alone with a group of children while qualified teaching staff had stepped away from the classroom.

Based on LPAs observations and interviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.

An exit interview was conducted, Appeal Righs Discussed. LPA provided the Licensee with a copy of this report

A NOTICE OF SITE VISIT WAS ISSUED AND LPA VERIFIED THAT IT WAS POSTED IN A PROMINENT LOCATION AT THE FACILITY BEFORE LEAVING. THE DIRECTOR UNDERSTANDS THAT IT MUST REMAIN POSTED FOR THE NEXT 30 DAYS ALONG WITH A COPY OF ALL TYPE A DEFICIENCIES (LIC9099D) CITED DURING THIS INSPECTION. A COPY OF ALL TYPE A DEFICIENCIES CITED DURING THIS INSPECTION MUST ALSO BE IMMEDIATELY (WITHIN 24 HOURS OF THE CHILD’S NEXT DAY IN CARE) GIVEN TO THE PARENTS OF ALL CHILDREN ENROLLED IN THE CHILD CARE FACILITY AND ANY CHILDREN ENROLLED INTO THE CHILD CARE FACILITY OVER THE NEXT 12 MONTHS (AT THE TIME OF ENROLLMENT.

A copy of this report was provided to the licensee on this date and must be made available to the public upon request for the next 3 years.
