Licensing Program Analyst (LPA) Rachel Zeron arrived at the facility to conclude a complaint investigation in regard to the above allegations received by Community Care Licensing (CCL) on 04/26/2023. A previous inspection was conducted on 04/28/2023 to initiate this investigation. LPA were given access to the facility by the director, Margaret Gale Van Duzer. LPA discussed the purpose of today’s visit, took census, and toured the facility. LPA met with the director to further discuss the complaint allegations and deliver findings.

During the investigation, LPA made observations, reviewed pertinent documentation, and conducted interviews with pertinent parties. It was alleged, facility staff are not ensuring that day care children are receiving proper meals and facility staff do not meet day care children's diapering needs also resulting in rashes. LPA conducted interviews with pertinent individuals and found that interviews were contradicting, some revealing that children are left in a soiled diaper at pick up and others revealing that the children are changed timely and there are no signs of diaper rash. Staff interviews indicated that the children are checked every 30 minutes to an hour.

The complaint alleged that children are dropping their food on the ground and the staff are picking up the food of the ground and throwing it away. The staff is then not replacing the food that is dropped. Staff interviews indicated that the facility has plenty of food and the food is replaced if the child is still hungry but also indicated that the children tend to throw their food on the ground or begin to play with it once they are not hungry anymore. Staff indicated they will ask the children if they would like more food.

Based on information obtained during this investigation and through interviews conducted, the review of pertinent documentation, and Interviews revealing conflicting information, the allegations are UNSUBSTANTIATED. A finding that the allegation is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the allegation occurred.

An exit interview was conducted with the director, Margaret Gale Van Duzer . Appeal Rights were discussed and issued, a copy of this report was provided, and a Notice of Site (NOS) Visit was issued.

The Notice of Site Visit (LIC9213) shall be posted where the parent/guardian of children enter and exit the facility. The Notice of Site Visit (LIC9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00

A copy of this report must be made available for the next three years
