On 06/05/2023 at 8:55 AM Licensing Program Analyst (LPA) Susan Brewer arrived at the facility to conduct an annual inspection. LPA was greeted by Licensee Stephanie Rojas and granted entry to tour the facility inside and out. LPA reviewed records and observed and/or discussed the following: The licensee Stephanie Rojas, has applied to increase their capacity to a Large Family Child Care Home, and the home was assessed by the LPA for the increase of capacity along with the annual inspection. Present were 1 adult resident and 1 minor resident.

Normal days and hours of operation are: Monday through Friday, 6:30am to 6:00pm
OFF-LIMIT AREAS INCLUDE: Garage, Entire 2nd floor, and east side of backyard

The inspection consisted of reviews of the following domain: Physical Plant, Care and Supervision, Records, Facility Administration, Staffing Ratio and Capacity, Personal Rights. The inspection found the facility to be in compliance in these domains, except as noted on the LIC809D.

The facility is operating within the licensed capacity and appropriate ratios. A census of 2 children in care.
· The Licensee is present in the home and has ensured that children in care are supervised.
· When temporarily absent from the home, the Licensee shall arrange for a substitute adult to care for and supervise children
· A working telephone is present.
· A fully charged fire extinguisher (2A:10BC) was observed. A smoke detector and carbon monoxide detector were present and tested by the Licensee during this inspection.
· All hazardous items are NOT made inaccessible, this includes detergents, cleaning compounds, medications and other items which could pose a danger to children.
· Storage of poisons is inaccessible to children and locked.
· There is a properly barricaded fireplace.

· Guns or weapons are present as stated by the Licensee Stephanie Rojas. During todays inspection, at 9:29 AM the LPA Susan Brewer, observed a gun cartridge clip, filled with ammunition and stored in an unlocked drawer, behind an unlocked door to the room. The LPA did verify that the fire arm was locked separately and in compliance with Title 22 regulations. No children were present in the room where the ammunition and firearm were stored. The licensee Stephanie Rojas, was reminded that all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 regulations.
· Stairs are barricaded at this time.
· Home is clean and orderly, with heating and ventilation for safety and comfort.
· Clean, Safe and age-appropriate toys and equipment are present for indoor; however, the LPA observed 2 play pens to be dirty, and some outdoor toys and play equipment to be dirty with dust, debris and webs, damaged and broken. No children were observed in the areas observed in the outdoor play area. In addition, the LPA observed miscellaneous outdoor landscaping rusty rebar to be sticking up from the ground, rusted hardware posts and chains, meant for anchoring a swing set to be loose and above ground.
· Outdoor play areas are fenced and/ or appropriate supervision is present.
· Verification of control of property on file.
· Property owner/landlord notification and consent: NOT ON FILE FOR A LARGE CAPACITY.
· Pediatric CPR and First Aid Card expire on 10/2023
· Health & Safety Certificate - completed on 10/2021
· Mandated reporter General: 06/05/2023; AB 1207 Child Care Expires: 08/2023
· Fire clearance: 05/25/2023
· Documentation of fire & earthquake drills to be conducted every six months: Last fire and earthquake drills on 04/06/2023, with 6 children present and lasted 3 minutes.
· There are no bodies of water, at this time. Licensee understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Children’s files are NOT complete, as 1 day care child present and enrolled at 8 months of age, did not have the LIC9227 individual sleep plan or sleep logs on file.
· Staff’s files are NOT complete. The licensee is in the process of hiring a qualified assistant.

· A review of staff records on 06/01/2023 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

Licensee Stephanie Rojas, was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA Susan Brewer, discussed the safe sleep regulations with licensee Stephanie Rojas, and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource.

LPA also informed licensee Stephanie Rojas, of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Before licensure, the following needs to be corrected/completed:
1. Property owner/landlord notification and consent for a LARGE CAPACITY. Only required if operating up to 14 children. 07/05/2023
2. Infant Safe Sleep log for infant under 2 years of age and written statement of understanding, 06/07/2023.
3. Clean play pens indoors and outdoors, submit photo by 06/15/2023.
4. Clean outdoor toys, equipment and clear outdoor area of broken toys and hazardous play equipment and hardware. Stabilize posts for swings and remove or cover protruding rebar from the ground along the perimeter of the yard. 06/15/2023.
5. Written statement of understanding the regulation for lock firearms and ammunition and or other hazardous items, 06/05/2023.

Once all corrections have been made, with proof sent to licensing, the application for a Large Family Child Care Home will be submitted for approval with a maximum capacity of 12, or 14 with parent notification. As agreed, upon by the licensee Stephanie Rojas, all corrections are due within 30 days. If not received within 30 days from the date of this report, the application may be withdrawn, and the license will remain a Small Family Child Care Home.

Civil Penalties were not issued this visit.

A notice of site visit was given and must remain posted for 30 days.
