On 1/23/2024 at 8:10 am, Licensing Program Analyst (LPA) Patricia Berry conduct a case management- incident. LPA met with Dianne Talens and was granted access into the facility. LPA toured the facility and took a census.


A case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 12/7/2023.

Facility records were reviewed, and staff were interviewed.

Further information will be needed. Upon completion of the review, the outcome and/or recommendations will be provided to the licensee.

An exit interview was conducted and a copy of this report was provided to director and appeal rights provided.

Notice of Site Visit issued.

The incident was reported to CCL and the parent.
