On 5/23/24 at 9:01 Licensing Program Analyst (LPA)) Patricia Berry conducted a case management visit. LPA met with Ashlee Stuehrmann and was granted access into the facility. LPA toured facility and took a census.

During the course of a complaint investigation it was discovered through staff interviews on occasion the preschool classrooms are out of ratio. Staff stated once the classrooms are out of ratio, it could take up to 30 minutes for additional staff to arrive.

Based on the information received the preschool classrooms are out of ratio.

See 809D for deficiency.

Exit interview conducted with director, appeal rights and Notice of Site Visit issued.

Notice of Site Visit must be posted for 30 days.
