On 08/01/2024 at 9:00 AM, Licensing Program Analyst (LPA) Tiffanie Diep arrived at the facility to conduct a change of location inspection. Applicant Sandra Figueroa is requesting a change of location from previous facility (#364845755) to current location. LPA toured inside and outside of the home, and LPA was accompanied by Applicant during the inspection. Applicant’s spouse (S1) and three minor children (C1-C3) were also present during the inspection. LPA observed and/or discussed the following:
  • Tentative days and hours of operation are Monday through Saturday from 6:00 AM to 6:00 PM.
  • Off-limits area includes: the entire second floor.
  • An appropriate fire extinguisher was present. A functioning smoke detector and carbon monoxide detector were present and tested by Applicant during the inspection.
  • All hazardous items were stored inaccessible to children.
  • Toxins were locked.
  • There are no weapons present in the home per Applicant. Applicant understands all firearms, weapons, and ammunition must be locked separately and made inaccessible to children in care according to Title 22 Regulations.
  • Stairs were barricaded by a baby gate.
  • Fireplace was properly screened to prevent access by children.
  • Applicant provided proof of control of property (Mortgage Statement).
  • Facility sketches, Emergency Disaster Plan (LIC 610A), and Notification of Parents' Rights poster (PUB 394) were posted.
  • Preventive health and safety training including nutrition and lead exposure was completed on 07/21/2024.
  • Pediatric CPR and first aid certification expires on 10/18/2025.
  • Mandated Reporter Training certificate expires on 10/25/2025.

  • There is a fountain located by the front door which was not running during the inspection. Applicant stated the fountain is inoperable. Applicant understands all bodies of water, including in-ground and above-ground pools, hot tubs, spas, and ponds, must be inaccessible to children in care and be properly covered or fenced according to Title 22 Regulations. The Department must be notified prior to installation of these and similar bodies of water.
  • Clean, safe, and age-appropriate toys were present.
  • LPA observed a trampoline in the backyard.
  • Applicant was informed of their reporting requirements and was provided with the Regional Office’s Unusual Incident Reporting e-mail at UnusualIncidentReportsDO09@dss.ca.gov.
  • Applicant can submit transfer forms to associate new individuals or to disassociate someone from their facility via e-mail to Associations_Disassociations862@dss.ca.gov.
  • The Duty Officer is available to answer questions Monday through Friday from 8:00 AM to 5:00 PM at (951) 782-4200.
  • Resident and/or staff records reviewed during today’s inspection indicate that all adults who require caregiver background checks have received all required clearances or exemptions.

The following was discussed with Applicant:
  • Maintain a current phone number on file with the Regional Office.
  • Constantly reassess the inaccessibility of hazards.
  • Complete mandated reporter training as specified on their mandated reporter duties and renew the training every two years.
  • Conduct and document fire and disaster drills at least once every six months.
  • California Code of Regulations section 102417 prohibits baby walkers, bouncers, jumpers, and similar items.
  • Applicant was encouraged to visit Community Care Licensing Division’s (CCLD) website at http://ccld.ca.gov/ for information regarding child care updates, forms, regulations, and legislation pertaining to Community Care Licensing.

LPA reviewed with Applicant the Forms/Records To Keep In Your Family Child Care Home (LIC 311D), children’s forms/records, facility forms/records, and information to be posted. Entrance Checklist (LIC 9280) was provided to Applicant.

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of five days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with Applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

CCLD regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters, and other important information communication platforms. To receive important licensed-related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive e-mail communication.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

On 06/10/2024, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders (RSOs) were found at the facility addresses. Under state law, some RSOs are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all RSOs against all Community Care Licensing Division (CCLD) facility addresses pursuant to information shared by California DOJ.

A license for a small Family Child Care Home is granted effective today, August 1, 2024. The facility is approved to serve a capacity of 6 children with no more than 3 infants, or 4 infants only, or up to 8 children with no more than 2 infants, 1 child enrolled in transitional kindergarten or above, and 1 child at least age 6. Infants are children under the age of 2.

An exit interview was conducted and report was reviewed with the applicant, Sandra Figueroa.
