On 04/07/2025 at 10:00 AM, Licensing Program Analyst (LPA) Raymond Moorehead arrived at the facility to conduct a pre-licensing inspection. Present during this inspection was Yifan Zhou, Applicant. LPA toured the facility, inside and out and the following was observed and/or discussed:

Normal days and hours of operation are: Monday through Friday from 6:00 AM to 6:00 PM.

OFF-LIMIT AREAS INCLUDE: North (left) side of backyard/pool area, entire second floor, downstairs bedroom/storage room, kitchen, and garage.

· Appropriate fire extinguisher, smoke detector and carbon monoxide detector are present and were tested by the applicant during this inspection. Fire extinguisher, smoke detector and carbon monoxide detector are in working order.

· All hazardous items are made inaccessible

· Toxins are locked

· Firearms are not present in the home as of this date according to Applicant. Applicant understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations.

· Stairs are properly barricaded to prevent access to children

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

· There are no toxic plants observed at this time

During today's visit, LPA observed two different bodies of water at the home. The first body of water that was observed was a fountain that was measured to be 1 foot deep. The fountain was observed in the backyard. The second body of water that was observed during today's visit was a in-ground pool that is located on the left side of the backyard (if you are facing home from the front) ; and leads to the front of the house/driveway area.

The fountain was observed to have a body of water, and is surrounded by a wooden fence that was measured at 2 feet and 9 inches. The front of the in-ground pool is fenced off by a roth iron gate that was measured to be 4 feet and 9 inches. Please note that the roth iron fence is self closing and self latching. Other than the roth iron fence, the pool is surrounded by a brick wall that measures to be 5 feet and 8 inches. If you are facing the pool from the roth iron fence's entrance, the left side of the pool area is protected by a brick wall. The brick wall obscures the pool from view, if you were on the outside of it.

The area on the other side of this brick wall is the home's driveway.

In regards to pools, Title 22 regulation 102417 (5) (A) states that fences shall be at least five feet high and shall be constructed so that the fence does not obscure the pool from view. The bottom and sides of the fence shall comply with Division 1, Appendix Chapter 4 of the 1994 Uniform Building Code. Assembly Bill 2866 states that

Applicant was informed that all bodies of water including ponds, above ground pools and spas, in-ground pools and spas, and some fountains must be properly covered or fenced per title 22 regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position when not in use.

Applicant was also informed that the pool regulation requirements also include a life ring with a minimum exterior diameter of 17 inches (and labeled as approved by the United Sates Coast Guard) and a rescue pole with a body hook and minimum fixed length of 12 feet.

Under HSC section 1596.814, these facilities must also perform a daily inspection of their drowning prevention safety features and safety equipment before opening the facility, and they must maintain a log of the inspections to be provided to the department upon request.

Applicant was also informed that if the home's fence does not meet regulation requirements, she may have a pool cover instead. The pool cover must meet the performance standards of ASTM International Standard F1346-23. Licensee must maintain and make available for the Department’s inspection, documentation verifying that the pool cover meets this standard. Applicant was also informed that with the pool cover, she must also have a pool alarm that will sound upon detecting an entrance into the water and must be turned on and be in working condition during the family child care's operating hours while the swimming pool is not in use.

LPA provided Applicant with a copy of PIN 25-01 and Assembly Bill 2866 and reviewed the details.

- AB 1207 – Mandated Child Abuse Reporting: Child Day Care Personnel Training, beginning January 1, 2018 – In accordance with California Health and Safety Code Section 1596.8662 – requires all licensed providers, applicants, directors, and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Applicants must meet requirements as a precondition to licensure. Existing licensees must meet requirements by March 30, 2018. New employees shall have 90 days to complete training as required. This training requirement may be directly met by using the Department’s Office of Child Abuse Prevention (OCAP) online training modules. The OCAP modules are free of cost and available at: http://www.mandatedreporterca.com/ and are provided in English and Spanish. If no training is made available in a required person’s primary language, then those persons shall be exempt from this requirement.

AB2370 – Effective January 1, 2019 – Lead Poisoning – providers are required to provide a lead toxicity prevention handout to parents/guardians of newly enrolled and newly re-enrolled children with information on risks and effects of lead poisoning; blood lead testing recommendations and requirements; and options for obtaining blood lead testing, including free and/or discounted testing. There will be a training component of this added to the Preventative Health Training beginning July 1, 2020.

LPA discussed the safe sleep regulations with Applicant Yifan Zhou and discussed the Child Care Licensing Safe Sleep webpage at:

LPA also informed applicant, of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Applicant Yifan Zhou was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Before licensure the following needs to be completed:

1.) Compliance with all pool regulations

- Applicant agrees to come into compliance with Title 22 pool regulations.


Once all corrections are ready, Licensing Program Analyst will conduct another inspection in order to verify corrections made by Applicant. Once all corrections have been verified, the License will be granted.
