On the above noted date and time. Licensing Program Analyst's (LPA's), Diana Brasel and Griselda Castellon conducted an announced 2nd follow-up Pre-Licensing inspection to add an infant component. An initial Pre-licensing visit was conducted on 9/11/2025 with director/owner/licensee Feng Xue and owner/licensee Wei Zhao. Owner/licensee Wei Zhao on 9/11/2025 was not present for most of the visit and was not present for today's visit. On the initial visit the infant napping area was not completed or the infant playground. The licensee's had requested rooms 5 and 6 to be infant rooms and taking the two rooms away from their current preschool license and capacity. On initial visit the request was changed to room 5 only to be the infant room and to leave room 6 as part of the current preschool rooms and capacity. The licensee's originally requested to have 26 infants ages 0-2 and 90 preschool children ages 2-5.
An updated LIC 200A was requested during the initial visit to reflect the current updated request.
An updated LIC 200A dated 09/24/2025 was submitted via email with the updated request being 15 infants ages 0-2 and 101 preschool children ages 2-5. An updated facility sketch was also submitted showing room 5 to the infant room and rooms 1, 2, 3, and 6 to be preschool rooms.
On todays visit an updated LIC 200A was obtained to reflect the amount of children that can be requested for preschool due to indoor activity measured space.

The facility currently has a preschool license # 364846174, upon completion of adding the infant component the facility will be converted into a single license. Hours of operation will be Monday - Friday 7:00 am - 6:00 pm.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

An updated STD 850 was emailed to the facilities assigned fire department on 10/01/25. A granted fire clearance with the updated STD 850 shall be received as part of needed corrections prior to licensure to add the infant component.

All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Feng Xue director/owner/licensee that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Feng Xue stated that they will be in the teacher break room. A fully equipped first aid kit is in each classroom. There is an operational carbon monoxide detector on site located in each classroom. All required licensing documents were observed posted in the front office area. Children will be signed in and out at the front office area.

On today's date the indoor activity space for infant room 5 and preschool room 2 were measured, rooms 1, 3, and 6 were measured during the initial visit. Total indoor activity space measured for infant room 5 is 545.24, which is sufficient to accommodate the requested capacity of (15) children. On today's date the indoor activity space for preschool rooms 1, 2, 3, and 6 measured 3011.37, which is sufficient for 86 children. The licensee requested as of 9/24/2025 on the most recent received LIC 200A 101 preschool children. An updated LIC 200A was requested today to show 15 infants and 86 preschool children. LPA's observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via filtered bottle water. The children bring their own water bottles and the facility has disposable cups if needed. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property.

The facility currently has a fully fenced playground area which is separate from other components. Fencing wrought iron and block wall, which is at least four feet high. The total square footage for the outdoor activity space for infants is 462.50, which is insufficient for the number of children being requested. A playground waiver to share the playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure. Shade is provided via an easy up. The total square footage for the outdoor activity space for preschool is 7610.23, which is sufficient for the number of children being requested. Shade is provided via wind sails and patio wooden cover extension from the building for preschool There are sufficient outdoor age-appropriate toys and play equipment available on the preschool playground. There is a climbing structure located on the preschool playground which is age appropriate, anchored properly and previously cleared upon licensure of the preschool program. There is adequate cushioning in fall zones of climber provided by rubber wood chips. Drinking water is available via children's filled water bottles with filtered drinking water. LPA observed all hazardous items on the playground to be inaccessible to children. The infant playground had no outdoor play equipment or toys on today's visit. (see correction needed) Feng Xue Licensee/Director/Owner was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

On today's visit the infant playground fencing has been completed and is 4 feet tall in all areas except the upper entrance area located on the sidewalk, which measured less than 4 feet. (see correction needed). The infant playground had no age appropriate toys or equipment on today's visit. (see correction needed).



Per the licensee/owner he has not been required to have a lead test for the facility. The facility serves bottle filtered water only, has no drinking fountains and does not have a kitchen.

The facility has stated they will be using baby base 2-in-1 booster feeding floor seats for high chairs, the facility has 5 present on today's visit. The plastic seats are in good repair and trays lock onto the feeding seats. The seats are made of washable, moisture-resistant material.

Changing table has at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink. Diapering sink is not used for meal preparation or dishwashing. Cribs meet regulatory requirements. Crib area is currently separated from activity space, the gate has not been secured to lock or latch. (see correction needed). The barrier is at least four feet high, made of sound absorbing material. Direct supervision at all times will be required for napping children. Sleep logs will be kept infant room. Needs and Services Plans and Sleep Plans will be kept in the infant room. The facility has a total of 6 cribs, the director/owner/licensee understands no more than 6 children (ages 0-12 months) may be present on a daily basis.

LPA's observed a total of (2) sinks, (1) toilet, (1) potty chair, and (1) changing table available for infant children’s use. LPA's observed (7) sinks and (7) toilets available for preschool children's use. These are sufficient to accommodate the requested capacity of (15) infant children and (86) preschool children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the front office located near staff restroom. There is at least one potty chair or toilet for every five infants.



Facility will provide am and pm snack to preschool children only. Parents/Guardians are responsible for bringing their child's lunch, the facility has available purchasing of lunches which they will order and have delivered. The options are on the posted lunch menu. The facility does not have a kitchen snacks are stored in the storage/teacher break room. Infant parents/representative will be required to provided their children's lunch, am and pm snacks daily as stated in the parent handbook .

Director/licensee/owner was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Infant playground:
1. Proof that the infant outdoor playground is equipped with a variety of age-appropriate toys and equipment.

2. Proof that the fence area on the walkway has been increased to measure 4 feet tall.

Infant room 5.

1. Proof that the infant napping entrance door has been locked or latch.

2. Proof that the chipped baseboard located near entrance door to room has been repaired to prevent access to the compressed wood.

3. Proof that the two areas located on the ceiling where wet spots were visual and in one area located near the infant changing table/adult sink, water was dripping from the ceiling to the floor during todays while raining. Please provide proof request for repairs to the landlord.

The updated granted STD 850 shall be obtained prior to licensure of adding the infant component.

All needed missing documents needed for a complete application were obtained and reviewed at time of visit.

Exit interview conducted and report was reviewed with the Director/licensee/Owner.

LPA's discussed additional resources during the visit and LPA Brasel will email the discussed resources to the facilities email.
