On February 2, 2023, Licensing Program Analyst (LPA) Kuliema Calloway, met with Licensee for the purpose of conducting a Case Management Inspection Visit. Licensee is requesting to provide care and supervision for a Large Family Child Care Home for the capacity of 14 children. LPA met with Licensee who granted access. LPA observed two kids in care and one staff. Licensee would like to change the hours of operation to: 6:00 AM to 5:00 AM. For Monday through Sunday and overnight care.
The Home is set up as follows:
This is a single-story home with three (3) bedrooms and two (2) bathrooms, a kitchen, living room, laundry room, and garage. Per the Applicant the following areas of the home will be utilized for the Family Child Care: The living room which is located on the left-hand side once you enter the front door. The home was inspected for safety, comfort, cleanliness, telephone service, central air, and heat, and ventilation. The home has central heating and air conditioning. All windows have screens and are free of cracks, bugs, and debris.
Main Care Area: Childcare is provided in the living room. LPA observed age-appropriate toys and furniture for the children that is in good repair. There are toys and books on the premises of this facility. The area is neat and clean. There is a wood burning fireplace inside the area that is gated and made inaccessible to the children. All unused electrical outlets are covered and made inaccessible to the children.
Children's Bathroom (#1): Children will use Bathroom #1 which down the hallway from the childcare area and next to Bedroom#1 (off limits). The Bathroom was toured and inspected sink/toilet are in operable condition. Toilet and faucets are clean, safe, and operable. There were no shampoos, mouthwash, razors, poisons or medications observed. The cabinets and drawers were locked with safety latches and made inaccessible to the children. The tub/shower were locked with safety latches and made inaccessible. The bathroom was clean, sanitized, and in good repair.

Kitchen: The kitchen is accessible. The kitchen was inspected there were no hazardous or dangerous items observed in the upper drawers. The lower cabinets had pots and plastic containers were secured with safety latches. In the kitchen, all sharp utensils were stored inside the kitchen pantry which has a safety latch on the door. The upper cabinets have glasses, plates, and are accessible to children. LPA suggested safety latches. The eating area for the smaller children was located inside the kitchen. Dining seating for at least two children were counted. LPA suggested larger capacity table and chairs to accommodate 14 children. The refrigerator and freezer were inspected, and both were clean and in good repair.
Backyard: The backyard was inspected and is (Off Limits). The backyard is fenced all around. There is a swing and slide set, but the children will not play in the backyard. The right side has a gate that accesses the side of the house with a padlock. The left side of the backyard is not used for childcare and is off-limits for the children in care. There is a gate with a padlock to enter. There are several pets in the yard. There is a concreted area and dirt area.
Off Limit’s Areas: Bedroom #1, Bedroom #2, and Bedroom #3, LPA observed safety knobs on the doors, the laundry room has a key locked door. There is an added Garage that is separate from the home and off limits.
Others:
AC/Heating Unit was observed. The AC/Heating Unit is on top of the home and inaccessible to the children.
Electrical outlets: All unused electrical outlets have safety plugs and are made inaccessible to children.
Fire extinguisher (2A10BC): LPA observed there is a required fire extinguisher (2A10BC) that is FULL and reading in Green and located inside the locked pantry and made inaccessible to children. It meets standards established by the State Fire Marshall.
Fire Push and Pull- Down Switch: Observed with a key for turn on and off function located on the left wall once you enter the front door.
Fireplace: The fireplace was observed in the living room (childcare area) and has a screen/glass barrier with a gate in front to make it inaccessible to the children.
Hanging window blind cords: The cords are tied and up high so that they are inaccessible to children.

Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to the publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm. The Licensee does not provide IMS to the children at this time.
Isolation area (Illness): Per the licensee, if the child shows signs of illness, he/she will be separated from other children and stay in the napping room on a cot until the parent can pick up the child.
Medications and cleaning solutions: Detergents/cleaning compounds are in kitchen cabinet with safety latches and are inaccessible to the children. LPA suggested moving to the locked laundry room as capacity increases.
Napping: Children will nap in the designated nap areas with adult supervision. LPA observed two (2) cots inside a locket closet in the childcare area. Licensee will increase cots with capacity increase.
Overnight Care: Per licensee, overnight care will be provided. Licensee is aware the care is for twenty-three hours.

Pets: LPA observed seven dogs, one dozen chickens, one goat, and one pony. The dogs were observed running around in the yard unleased and uncaged. The chickens, goat and pony, were behind a fenced, caged area. All of the animals were in the off-limits back yard and according the Licensee, the children will not play with the animals.

Phone service: There is not a working landline. Licensee and Assistant both have a working cell phone.


Smoke Detectors and Carbon Monoxide Detectors: There were three (3) smoke detectors one in the kitchen, Bedroom #3, and in the hallway. There was one (1) carbon monoxide detectors on the wall near the front entrance. All devices were tested and are operable.
The First Aid Kit: There were two first aid kits. Both were inside the kitchen in a lower cabinet locked with a safety latch and made inaccessible to the children. The First Aid Kits were observed with supplies, but none of them had the First Aid manual. LPA suggested to obtain a manual for instructions. r
Transportation: The licensee will provide transportation for the children. The vehicle registration and the insurance
Weapons or Firearms: Per Licensee, there are No Firearms currently in the facility at this time. LPA did not observe any firearms.
Hot Water: Was tested at a safe temperature between 105-120 degrees.

Documentation:
CPR/First Aid: LPA observed licensee and licensee’s assistant has current Pediatric CPR and First Aid Training with expiration date of 10/2023 Eight (8) hours of Preventive Health and Safety Training completed 7/13/2019.
Mandated Reporter Training: Applicant completed the required training on 3/27/2022 and Assistant’s expired on 10/4/2022.
Criminal Record: Per Guardian, all adults who live in this facility obtained a Criminal Record Clearance.
Facility fees: Per Licensing Information System (LIS), facility fees were current.
Emergency Disaster Plan: LIC 610 A was submitted and received by CCLD.
Immunization: The licensee has the required immunizations: Negative TB skin test, MMR and DTAP) and Influenza.
Valid Driver License: Applicant has a valid California Driver License with an expiration date of 11/09/2027. Assistant: CDL expires: UNKNOWN date, will provide.
Infant Sleeping Plan (LIC 9227) and Sleeping Log: LPA shared the Safe Sleeping Regulation with the licensee.
LPA advised licensee to post as required: Emergency Disaster Plan, Earthquake Preparedness, and Parent's Rights Poster.
The following information was discussed with the licensee:
Mandatory Forms for the children’s files and provider’s files.
Requirements for fire drills, earthquake drills, and documentation for both.
The licensee is reminded that 100% supervision is always required for all children.
Capacity limits, parent/child roster maintenance, posting facility license, knowledge of Emergency Plan, and documenting Disaster Drills (Fire and Earthquake).
All food is properly labeled if brought from child’s home; and all food is checked periodically for expiration dates.
Applicant was advised how to access forms and Regulations for Family Child Care online at: www.ccld.ca.gov.
Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must always have the facility’s phone number; if the phone number is changed, licensing must be notified.

A licensed child care provider, administrator, or employee of the licensed child day care facility shall complete an on-line Mandated Reporter Training and shall renew the training every two (2) year at: www.mandatedreporter.com
Licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form (LIC 624B).
The licensee was informed of the responsibility to report suspected Child Abuse by calling the Child Abuse Hot-line at 1-800-540-4000. Also, call the CCLD office and follow up with a written Unusual Incident/Injury Report (LIC 624B).
Criminal Record Statement: Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a Criminal Record Clearance or Exemption, or transfer their existing clearance or exemption, prior to the initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Safe Sleep: LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Notice of Site Visit: A notice of site visit must remain posted for 30 days.
Posting Requirements: Failure to comply with posting requirements shall result in an immediate civil penalty of $100.00.
Parent Notifications Requirements: Per the Health and Safety code (HSC) 1596.859, 1596.8895, and 1597.05 to improve transparency of licensing records and to ensure that the parents/guardians using the licensed childcare facility are aware of situations that present danger to the children. Employees or volunteers at family day care home; immunization requirements; records; exemptions (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against Influenza, Pertussis, Rubella, Mumps, and Measles. Each employee and volunteer shall receive an Influenza vaccination between August 1 and December 1 of each year.

Type A citation: Type A citation(s) shall be posted for 30 consecutive days along with the Notice of Site Visit Letter (printed out after every visit) and posted during hours of operation, as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Licensee shall provide a copy any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Failure to do so will result in a Civil Penalty being assessed.
The regulation prohibits the smoking of tobacco in a private residence that is licensed as a family childcare home and in those areas of the family day care home where children are present (24/7 ban). State law prohibits baby walkers, bouncy seats, exer-saucers, and any other items that fall into that category.
Licensee was advised to visit the CCL website (www.ccld.ca.gov) to obtain updates of courses and updates/changes to the regulations.

· Copy of Assistant (Daughter’s) Driver License.

**As a result of this inspection, the home does not meet the Title 22 Regulations for capacity increase at this time. Corrections are required.
