An exit interview was conducted, a copy of this report was read, and a Notice of Site Visit, and Appeal Rights, were provided to Magdalena Galvan, Licensee. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain the posting may result in $100 civil penalty.
