Licensing Program Analyst (LPA) Babatunde Ibitoye met with Antoinette Mcclendon, Owner/Director, today for the purpose of conducting a Annual/Random inspection for this Pre-School component with a capacity of 24 children. Center has also School-Age Component on the same property (#367750008). Owner/Director led LPA on a tour of the center which consist of 2 classrooms for the Pre-School and the outside area. The days/hours of operation: MONDAY THROUGH FRIDAY 5:00AM TO 8:00PM.Incidental Medical Services (IMS) were discussed

**Classrooms were equipped with age appropriate furniture, equipment, toys and materials. The classrooms were observed to be safe and free of any Health or safety hazards. Telephone service was verified as well as adequate heating, lighting, and ventilation. Children's belongings are kept in cubbies along the wall. Drinking water is available inside the classroom in the form of a water dispenser and cups.

**The children's bathrooms are located within the classrooms. Preschool Classroom #1 contains 1 toilet and 1 sink. Preschool Classroom #2 has 1 Staff bathroom with one toilet and one sink. In addition Classroom #2 has 2 bathrooms each with 1 toilet and 1 sink. The boys bathroom has an additional urinal. Across the hall from Preschool #1 is the staff room and bathroom. LPA observed the bathrooms to be clean and sanitary, with soap, toilet paper and paper towels readily available.All other toilets and sinks are functioning properly and age appropriate.

**Each classroom has cubbies, stackable mats, cots, linens and bedding stored individually so that each child's bedding is identifiable and no child's used bedding comes into contact with other bedding. Each mat and cot is equipped with a sheet to cover the mat and cot, bedding and linens are laundered weekly and as needed. Flooring was inspected for cleanliness, and good repair.

**Food preparation area/Kitchen (staff and center kitchen) was inspected for safety, cleanliness, proper equipment & protection against contamination and storage. Center provide breakfast, am snack, lunch, pm snack and dinner. Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children were stored and inaccessible to children.

**The Parent Board (located in the main entrance area) contained all documents that are required to be posted according to Title 22 Regulations. Staff are certified in Pediatric CPR and First Aid (exp 05/08/2025).

LPA observed a fully stocked first aid kit; fully charged fire extinguishers; carbon monoxide detectors throughout the center.· Requirements for fire drills, earthquake drills,No documentation for both see LIC 9102. The last fire/earthquake drill was conducted on 07/30/2024, LPA advised the Director to conduct a disaster drill every six months.
Health-Related Services: The Director has been advised all prescription and non-prescription medications must have the child’s name and are dated, written consent and instruction from the child’s representative, and a plan to document and report to the child’s representative when medication is administered to a child; Medication will be properly labeled and stored in its original container. At this time center will not be administering over the counter or prescription medication.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. All staff are required to take 3-hour Child Care Provider Mandated Reporter training (AB1207) every 2 years. Beginning on January 1, 2018, Assembly Bill 1207 (2015) requires all licensed providers, applicants, directors, and employees to complete training as specified in their mandated reporter duties and to renew their training every two years. Applicants must meet requirements as a precondition to licensure. New employees shall have 90 days from the date of employment to complete training as required. The director is aware self and all staff are mandated child abuse reporters and have the responsibility of reporting any suspected child abuse to the Child Abuse Hotline at (800) 540-4000.

**Outdoor play equipment was inspected for health, safety, good repair and age appropriateness. Center utilizes sand under climbing structure for cushioning material and a tree and patio for shade. There are 2 swing sets that are securely anchored, bike path, sand, little tike toys and large play tires. LPA observed the fencing surrounding the play area to be at least 4 1/2 ft. tall to protect children and to keep them in the outdoor activity area, the gates were locked during this inspections with safety latches. There are no bodies of water observed on the premises.

§1596.7995 (a)(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.



Center was made aware that prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review shall obtain a clearance or criminal record exemption. If a fingerprint clearance has been obtained through the Department, Licensee may request a transfer of a criminal record clearance from one state licensed facility to another using form LIC 9182.

Director advised of the requirement to report Unusual Incidents. Licensee was informed to utilize the Unusual Incident Report/Injury Report LIC624B when submitting the report to the department (email address on the website: www.unusualincidentreport@dss.ca.gov). A report shall be made to the department by telephone or fax during the department's normal business hours before the close of the next working day following the occurrence during the operation of the daycare center. In addition, a written report shall be submitted to the department within seven days following the occurrence of any events specified above. An On Duty Worker is available for questions at (661) 202-3318 Monday through Friday 8 am-5 pm.

Any duly authorized officer, employee, or agent of the Department shall, upon presentation of proper identification, shall inspect the facility. The director shall permit the Department to inspect the family child care home and to privately interview children or staff, to determine compliance with or to prevent violations of child care center or regulations, also enter and inspect any place providing personal care, supervision, and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation.

Upon receipt of a Type A deficiency the licensee shall post the report for 30 days in addition to the Notice of Site Visit & provide copies of the licensing report to parents/guardians of children in care. This report must be provided to parents/guardians of children newly enrolled at the facility during the next 12 months & the licensee will obtain a signed Acknowledgement of Licensing Reports (LIC 9224) from the parent/guardian & place it in each child's file. If these requirements are not met, civil penalties will be assessed.

An exit interview was conducted, and a copy of this report was provided to the Director Antoinette Mcclendon along with a Notice of Site Visit and Appeal Rights.
