On December 20, 2023, Licensing Program Analyst (LPAs) Calloway and Ali made an unannounced inspection to the Just 4 Toddler Preschool. The purpose of the visit was to deliver findings for the above allegation. LPA met with Asst. Director who granted access. LPA observed 6 children and 2 staff in care.

During the investigation, LPA conducted interviews with staff, parents, and all parties involved. Based on the interviews, the evidence revealed many of Child 1’s injuries were explained thus, it was inconsistent with the allegation. Therefore, the allegation is Unsubstantiated, meaning the allegation may have happened or is valid, but there is not a preponderance of the evidence to prove that the alleged violation occurred.

An exit interview was conducted, and a copy of this report was read, and a Notice of Site Visit, was provided to Ana Ruiz, Assistant Director, (S1) at the facility. A Notice of Site Visit must remain posted for 30 days. Removal of posting is subject to a $100 civil penalty.

On December 20, 2023, Licensing Program Analyst (LPAs) Calloway and Ali made an unannounced inspection to Just 4 Toddlers Preschool. The purpose of the visit was to deliver findings for the above allegations. LPA met with Asst. Director who granted access. LPA observed 6 children and 2 staff in care.

During the investigation, LPA conducted interviews and documented evidence. Based on the interviews with staff, parents, and all parties involved, the evidence revealed the center failed to notify Licensing of several biting injuries for Child 1, under Title 22 – Reporting Requirements.
Therefore, Allegation #2 is Substantiated, meaning the Preponderance of the evidence standard has been met.
There is one Type B deficiency cited during this visit. Per Title 22 Regulations, Division 12, Chapter 1, for Reporting Requirements 101212 (d)(1)(B).

Per Assistant Director, a training for staff will be conducted on Repoting Requirements and proof of the training will be provided to Licensing by the chosen POC date of 1/10/2024.

Based on interviews with parents, staff, and all parties involved, the facility did not report biting injuries for Child 1 that required medical treatment to Licensing by the specified timeframe of when they occurred.
