32 | Around 10:50am, LPA toured the facility with the Site Supervisor. The LPA and Site Supervisor walked into the outdoor play area designated for the preschool children and observed Staff 1 and Staff 2 providing care and supervision over 28 preschoolers. The Site Supervisor admitted during their interview that they were aware that the teacher-child ratio is not to exceed 12 children per teacher and acknowledged there were 28 children in the outdoor play area at one time. After the walk through was completed, the Site Supervisor moved four of the older preschoolers and placed them in a separate classroom. Staff 1 stated there is a classroom checklist, LPA observed the checklist and noted there were 30 children checked in for the preschool today, 6/28/2023.
Based on information obtained, observations, and interviews with relevant complaint parties, the Allegation 1 is deemed substantiated for exceeding the teacher-child ratio and a Type A citation will be issued. A finding of substantiated means that allegations were valid because the preponderance of the evidence standard has been met.
As a Type A deficiency has been cited, a copy of the citation and licensing report must be posted for 30 days.
The same report must be provided to Parents/Guardians and the Acknowledgment of Receipt of Licensing
Reports LIC 9224 must be signed by Parents/Guardians of all enrolled children and any newly enrolled
children in the next 12 months following the citation. If these requirements are not met, civil penalties per
violation will be assessed.
An exit interview was conducted, a copy of this report read and provided to the Site Supervisor, and the Notice of Site Visit and Appeal Rights was explained and given to the Site Supervisor. |
