25 | On 6/28/2023 at 10:20am, Licensing Program Analyst (LPA) Andrea Pittman conducted an unannounced visit at the facility to conduct a case management. Upon arrival, LPA was met by Site Supervisor Sonia Galvez and permitted entry into the facility. LPA observed 28 children in care with 2 staff providing care and supervision.
Around 10:50am, LPA toured the facility with the Site Supervisor. The LPA and Site Supervisor walked into the outdoor play area designated for the preschool kids and observed Staff 1 and Staff 2 providing care and supervision over 28 preschoolers. After the walk through was completed, the Site Supervisor moved four of the older preschoolers and placed them in a separate classroom. The Site Supervisor admitted during their interview that they did not know the capacity limitations of the facility’s license and believed it to be 24 children. The presence of 28 children exceeds the capacity limits for the center which is a Type A violation. An immediate civil penalty of $500 has been assessed for the first violation.
As a Type A deficiency has been cited, a copy of the citation and licensing report must be posted for 30 days.
The same report must be provided to Parents/Guardians and the Acknowledgment of Receipt of Licensing
Reports LIC 9224 must be signed by Parents/Guardians of all enrolled children and any newly enrolled
children in the next 12 months following the citation. If these requirements are not met, civil penalties per
violation will be assessed.
An exit interview was conducted, a copy of this report read and provided to the Licensee, and the Notice of Site Visit and Appeal Rights was explained and given to the Licensee. |
