Infant File and Staff File Review: LPA reviewed a total of 3 infant files and three staff files. Per LPA review, Child 1 (C1) needs and service plan was completed in 10.10.2023, and zero documentation was present showing an updated plan. Staff 1 confirmed there was zero additional paperwork for C1. Based on LPA review one of three infant files reviewed did not have an updated needs and service plan updated at least quarterly. A technical violation was issued. Per LPA review of the 15 minute safe sleep checks, the staff initials of the person who conducted each check was not documented on the following dates: July 1, 2024, July 2, 2024, July 15, 2024, July 16, 2024, July 17, 2024. LPA reviewed additional dates and observed the staff were initialling each 15 minute check. A type B citation was issued. LPA reviewed three staff files. Files contained the required documentation.

LPA and Facility representative reviewed the LIC 311A Records To Be Maintained At The Facility.
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for
lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-
CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives
outlined in PIN 21-21.1-CCP.
