On 3/03/2026 Licensing Program Analyst (LPA), Isabel Ortega met with Facility Director Tiffany Turner Boyce and conducted an Annual Random Inspection. LPA toured and inspected the facility in accordance with the facility sketch. During inspection LPA observed four Infants, seven Toddlers, 12 Preschool and 6 school age children Arrived at 2:20 p.m.) in care and eight staff providing care and supervision. Total of 29 children in care.

Facility is currently operating in four classrooms. Facility currently has a single license which serves Infants 0-24 months, toddlers 18 to 36 months, preschool 3 to 5 years and school age 5 to 12 years.

Facility’s regular operational hours are from 6:00 a.m. to 6:00 p.m.

Facility has all the required posted documents: Facility License, Notice of Parent's Rights Poster, Emergency Disaster Plan, car seat information, and Earthquake Preparedness Checklist.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

During the inspection LPA observed adequate teacher child ratio in each classroom. Care and supervision were evaluated and determine to meet the basic needs of children. A review of the sign in/out sheets were conducted to verify the current census of children.

The infants are provided with cribs and toddlers , preschool are provided with cots for napping. LPA observed the safe sleep logs to be completed every 15 minutes and LIC9227 for for 0 to one year of age are being completed by parent when rolling over developmental milestone is met follow by staff signature confirming rolling over of an infant was observed. Changing tables were observed to have a water sink arm distance reach in the classrooms. Parents provide, sheets, and blankets for children over one year of age and taken home once a week for washing. Parents also provide bottles for infants daily and are taken home for sanitizing and cleaning.

An isolation area was inspected, which is located in the Director’s office. Children have the option of rest time and quiet time when needed cots are provided. Age-appropriate sinks and toilets were inspected for availability and good repair. Toilets flush properly; Each restroom has adequate toilet paper, hand soap and paper towels available. Restrooms are cleaned, restock of toiletries by staff during the day and custodian after hours. Staff also disinfect and sanitize throughout the day.

The furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and AC ventilation were evaluated. LPA observed individual storage for children's belongings. The classrooms consist of filtered water from a black water dispenser in each classroom. Also, children have the option to bring their own water bottles from home. For outdoor play children have the option to refill their water bottles and children take their water bottles outside. Although, facility stated purified drinking water bottles are also available. Facility did not sample the water for lead, facility stated center does not utilize the water for food preparation and children do not drink the water. According to facility children bring their own water bottles. Written Directives of lead testing of water in licensed Child Care Center pursuant to AB2370 Chapter 676 Statues 2018. PIN-21-21.1 CCP- Facility is cited a B citation today.

First Aid kit with supplies and temperature thermometer was observed. The smoke detectors/ and carbon monoxide detectors were observed to be operating properly. The fire extinguisher (2A10BC) is reading in green; the Fire extinguisher was last serviced on 09/05/2025(light blue tag observed).

Trash cans with tight lids were observed. Food was inspected, and it was properly labeled, stored, and within expiration date. Refrigerator is clean and operating at the proper temperature. A refrigerator is adequately equipped, clean, and free from hazards. There is hot running water in the kitchen area.

Cleaning supplies are out of reach of children.

Emergency Drills are conducted every sixth month last drill was conducted and documented on 1/13/2026 at 10:30 a.m. Food allergies are noted and are posted in the kitchen and in each classroom when a child may have allergies. According to Director currently no children have reported food allergies or IMS plans. According to facility only preferences for food and drinks are requested for some children and are noted. Facility provides children with breakfast, lunch and afternoon snacks. The snack menu is posted in the main office, kitchen and in each classroom. Parents may request a tangible copy.

The playground is well fenced all around, and enclosed. There is adequate shade area for rest. According to facility there are no bodies of water on the premises, LPA did not observe any bodies of water during the inspection. According to facility there are no weapons on the premises and LPA did not observe any weapons during inspection.

Children's records were reviewed for completeness. Health History, Emergency contact and Medical Exams; Immunization Records are all in the children's file. The facility roster was up to date, and all staff have been fingerprinted and associated to the designated license number.

Facility has at least one staff member and the Director certified in Pediatric CPR/First Aid EMSA approved, which does not expire until February 02, 2028.

Childcare Provider Mandated Reporter (AB1207) training is dated 12/23/2025.

The following Incidental Medical Services (IMS) were discussed.
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. All staff are required to take the Child Care Provider Mandated Reporter (AB1207) training every 2 years. www.mandatedreporterca.com.

Upon hired date all staff are required to be immunized and show proof of immunization records according to Title 22 regulations.

Beginning on January 1, 2018, Assembly Bill 1207 (2015) requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years.
