25 | On 09/10/2024 at 10:00 am, Licensing Program Analysts (LPAs) Michelle Hood and Julieta Abrego made an unannounced inspection to follow up on an incident that was disclosed during interviews wherein a child two years old was discovered alone in front of the facility. There were 14 children with three staff on the playground. During today's inspection, LPAs interviewed three staff and Pastor.
During interviews with daycare parents and staff on a complaint received on 06/20/2024, it was disclosed a child was found by a female outside of the facility of 111 Orange Avenue, 92118 sometime between 8:00 am and 9:00 am. A daycare parent was approached while leaving the facility by the female asking about the child. The daycare parent immediately took the children inside the facility to a facility staff that were on the facility playground at the time. After review of the facility file, and interviews it was determined the Pastor John Roamer (acting director) did not report the incident to the Licensing Department within twenty-four from the time of the incident and the LIC 624B was not submitted within seven business days.
LPA Hood informed Pastor John Roamer that this report dated 09/10/2024, documents one Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Hood informed Pastor John Roamer to provide a copy of this licensing report dated 09/10/2024, that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. |
