On February 27, 2025, at 9:15 a.m., Licensing Program Analyst (LPA), Angela Nguyen conducted an unannounced Annual Required inspection. LPA met with Facility Representative, Olivia Cunningham and disclosed the purpose of inspection. Facility Representative stated Director, Rebecca Miller was on vacation. LPA was led on a tour of the inside and outside the facility by Facility Representative. This is a full day program which operates on a year round schedule following the Mountain Empire Unified School District schedule. Days and hours of operation are Monday-Friday from 7:00 a.m. -5:00 p.m. The following ratios were observed: Classroom (main room) serves children age 3 through 6 years. There were 15 daycare children present with two (2) Teachers, Olivia Cunningham and Yvonne Dugger. Classroom: 2's (serves children age 2 years): There were 8 daycare children present with one (1) Teacher, Leticia Ruiz.

All toilets and hand washing facilities were observed to be in safe and sanitary operating condition. LPA observed Ajax and Lysol wipes in a cabinet under the sink in the bathroom in the 2’s classroom assessible to children. Leticia put a child proof lock on the cabinet during the inspection. The floors in the facility were observed to clean and safe. Solid waste storage containers were observed to have tight-fitting covers and in good repair in Classroom (Main room). Classroom (2's) had a trash can with an open lid with food scraps. The children bring their own lunch. The school provides a snack to the children. Drinking water is available both indoors and outdoors. Children bring their own water bottles. LPA observed areas around high climbing equipment did have sufficient cushioning material (wood chips) surrounding the play structure and slide. Shade was observed in the outdoor activity space by the use of trees. The facility has a functioning combination smoke and carbon monoxide detector and a first aid kit that meet statutory requirements. The last fire drill was conducted and documented on 8/23/2023. LPA reminded Facility Representative that fire/ disaster drill shall be conducted and documented every 6 months.
Facility Representative stated that there are no swimming pools or other bodies of water on the premises. LPA did not observe any bodies of water. Facility Representative stated there are no firearms or ammunition allowed or stored on the premises.

Capacity and limitations as specified on the license are being maintained. Children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than twelve (12) children in care.

Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

At least one person trained in CPR and Pediatric First Aid is present when children are at the facility. The name of the Facility Representative and the fully qualified teacher designated to act in the Facility Representative’s absence has not been reported to the Department. LPA was unable to review facility electronic sign in /sign out procedures. Facility Representative was reminded that the person who signs the child in and out of the facility shall use their full legal signature and record the time of day. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles. 2 out of 4 staff members present did not have a valid Mandated Reporter training.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.

Facility Representative states she will submit an updated board of resolution, admission agreement, parent handbook. LIC 309, LIC 308, LIC 500, IMS plan and sign in/out sheets to the Department within 30 days.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

Two (2) Type B deficiency per California Code of Regulations, (Title 22, Division 12 & Chapter 1), is being cited on the attached LIC 809-D.

Two (2) Technical Violation was issued.

A Notice of Site Visit (LIC 9213) was given to the Facility Representative, Olivia Cunningham and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Licensee/Appeal Rights (LIC 9058) was provided. Exit interview conducted and report was reviewed with the Facility Representative.
