According to R1’s most recent Physician’s Report, R1’s primary diagnoses is schizoaffective disorder, bipolar type. During an interview with the Department R1’s responses were incoherent and R1 was unable to stay on topic regarding the allegation. An interview with an outside source familiar with R1 reported they have frequent symptoms of auditory and visual hallucinations and delusions. Interviews with facility staff corroborated R1’s history of auditory and visual hallucinations. Additionally, an interview with S1 denied the allegation. During LPA unannounced visits LPA observed S1 to be treating residents with dignity and respect.
The Department has investigated the allegation that R1 was sexually assaulted by S1. Based upon the information obtained during this investigation, it is determined that the preponderance of evidence was not met to support or corroborate these allegations and therefore deemed unsubstantiated.
An exit interview was conducted with Eveline Denton, Medical Representative, to whom a copy of this report and the Licensee’s Rights (LIC9058 01/16) were provided.
