25 | On 12/29/22 at 9:20am, Licensing Program Analyst (LPA) Vicky Williamson conducted an unannounced case management inspection to follow up on a self – reported incident report. Upon arrival, LPA Williamson met with Center Director Monica Collins. LPA discussed the purpose of the inspection and proceeded to tour the facility. The following ratios were observed in the Pre-K Classroom, there were seven children with one teacher, in the Preschool Classroom, there were nine children with one teacher and in the Discovery Preschool Classroom, there were seventeen children with two teachers, ages 2 through 5 years old.
On 12/19/22, the director self- reported an incident involving Child 1 (C1) sustaining an injury to the forehead after falling in the classroom on 12/13/22 at around 9:40am.
On 12/13/22, Staff 1 (S1) was sitting at a table in the classroom engaging in an activity with preschool children. S1 observed C1 sitting on a rug with four children reading a book. S1 reported that C1 laid their book on the carpet, got up and proceeded to run, tripping over their foot and falling and bumping their head on the middle shelf of a three- tier bookshelf. Staff 2 (S2) immediately comforted C1 and S1 immediately contacted Director Kendra Dedmon via walkie talkie. Director applied ice to the injury and contacted C1’s parent. There were two teachers and 18 preschool children in the classroom during the time of the incident.
Interviews were conducted with the director, staff, and a daycare parent. LPA reviewed staff records and Child Supervision Record (CSR). Based on LPA’s observation and evidence obtained, it was determined that C1 sustained an injury to the left side of their forehead that required medical attention. LPA observed that the bookshelf and rug were safe and in good repair. This incident was determined an accident.
No deficiencies cited. Exit interview conducted with Director Monica Collins. Director was provided LIC 809, Appeal Rights (LIC 9058) and Notice of Site Visit. Notice of Site Visit shall be posted for 30 days from today's date. LPA observed the Director post the Notice of Site Visit on the wall. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
