On 7/25/2024, at 12:31pm, Licensing Program Analyst (LPA) Vicky Williamson conducted an unannounced annual required inspection. LPA Williamson met with Director, Marcela Zavala and disclosed the purpose of the inspection. LPA inspected and toured the indoor and outdoor of the facility. Facility operates Monday through Friday from 6:00 am to 6:00 pm. The following ratios were observed: Infant Classroom had 10 infants present with three (3) teachers; Toddler Classroom had six (6) infants (ages 12 months to 23 months) present with one (1) teacher and one (1) aide, eight (8) additional infants were observed on the playground with one (1) teacher and one (1) aide.

Furniture and equipment are in good condition. Floors in the facility are clean and safe. Disinfectants, cleaning solutions, medications and other hazardous items were made inaccessible to children. Potty chairs, toilets and hand-washing equipment are in safe and sanitary operating condition. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and rodents. Solid waste storage containers have tight fitting covers and are in good repair. Playground equipment is in safe condition. The surface of the outdoor activity space is maintained in a safe condition. Areas around climbing equipment, have cushioning material to absorb falls. Facility has smoke and carbon monoxide detectors that meet statutory requirements. The last fire/disaster was conducted and documented on 7/11/2024.

Director stated that there are no swimming pools or other bodies of water on the premises. Director stated that there are no firearms or ammunition allowed or stored on the premises.

Breast milk and formula are provided by the parents along with bottles including the child's name and date. The facility also provides formula. The facility provides breakfast, lunch, and snack. Children bring their own water bottles and filter drinking water is available at the facility. Menus are posted weekly.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Capacity and limitations as specified on the license are being maintained. Children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than four (4) children in care. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility. The name of the facility representative and the fully qualified teacher designated to act in the facility representative’s absence has been reported to the Department. Facility utilizes electronic sign in/sign out procedures. LPA reviewed facility sign in /sign out sheets. Director was reminded that the person who signs the child in and out of the facility shall use their full legal signature and record the time of day. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. Individual Sleeping Plans and Infant Sleep checks were also review were observed to be complete. LPA reviewed a sample of staff files and observed files were complete with health screening and current documentation of completed mandated reporter training. Files for Staff (S1) were incomplete for immunization records for measles.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA discussed the safe sleep regulations with director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource.

LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.



Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations deficiencies are being cited. See the attached LIC 809D.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100. LPA observed Notice of Site Visit posted on the window in the lobby.
