On 1/26/2024 @ 1:25PM, Licensing Program Analyst (LPA) Nancy Diaz conducted an unannounced case management inspection in reference to 2 self-reported unusual incidents involving 2 children on two separate incidents involving injuries while using the play structure.

LPA toured the classrooms with Tammy Peters. Observed present today were 60 (napping) children in 7 classrooms. Appropriate staff-children ratios were observed in all the classrooms.

LPA observed that the play structure to be in safe condition. There were no parts protruding to cause the injury on one of the children.

Exit interview was conducted with site director Tammy Peters. Licensing report was reviewed and copy provided. Notice of site visit was also given. Notice of site visit must be posted for 30 days.
