On 10/19/23 at 12:30PM, Licensing Program Analyst (LPA) Patrick Ma visited the facility to conduct a case management site inspection. The purpose of this visit is to follow up on a self-reported incident that occurred on 10/16/23 regarding possible personal rights violation. Upon arrival, LPA met with Director, Melinda Tretera. Present at the facility were 20 daycare children and 5 staff. Facility was in ratio.

During the visit, LPA’s toured the facility, reviewed staff files, conducted interviews with children and staff, received children's and staff roster

Due to insufficient information available at this time, the alleged incident needs further investigation.

No deficiency was cited.

Exit interview conducted and report was reviewed with the facility representative Melinda Tretera. A notice of site visit was given and must remain posted for 30 days.
