25 | Licensing Program Analyst (LPA) Kelli Waters conducted a Case Management visit on 09/04/24 to follow up on an Unusual Incident Report (UIR) that was submitted to Licensing by the facility on 08/16/24. LPA met with Eunice Herrera, Site Director, and Cynthia De Dios, Program Director, to discuss incident.
The Director reported the following; on August 16, 2024, C1 was left unattended inside classroom B3 for approximately 4 minutes.
During interviews with staff and video footage of the incident, staff and children from classroom B3 were on the playground with 16 children and two staff. At 9:50am, C1 entered room B3 to use the restroom with S1 standing at the door providing supervision for both C1 inside and the remaining children outside. At approximately 9:51am, S1 leaves door to attend to children on playground. The door to room B3 closes and C1 is left unattended until 9:55am when S2 enters. S2 returned C1 to playground area.
Based on interviews and video footage, the facility is being cited for Title 22 Regulation Section 101229 (a)(1) Responsibility for Providing Care and Supervision on the attached LIC 809D.
Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility for the next 12 months.

An exit interview was conducted, and a copy of this report was provided.
The Notice of Site Visit and Type A Deficiencies from today’s visit must be posted for 30 days and a copy of this report must be made available to the public, at the facility site, for 3 years. |
