13 | On 12/11/25, Licensing Program Analyst (LPA) Saraliz Velando conducted an unannounced visit to deliver findings for a complaint received on 9/9/25. The LPA met with Program Director, Tamara Holthaus. Based on file review and staff interviews, there is evidence of a child/children biting, hitting, and exposing themselves to others. There is proof to show that these behaviors are occurring repeatedly and at times involving the same children. Staff interviews show that the staff has observed children discussing or naming body parts, but no staff was found to have witnessed actual touching. Staff and Parent interviews did not reveal any physical or emotional harm in children from being curious about their bodies however, there is concern about the frequency of the children engaging in biting, hitting, and exposing of private parts of the body. The training provided by the facility to the staff does not appear to curb the challenging behavior and meet the needs of the children in care. The preponderance of the evidence has been met and therefore, the above allegation is found to be SUBSTANTIATED. Type B Violation will be cited. Refer to page LIC 9099-D for the deficiency. The exit interview was conducted with the Director, Tamara Holthaus. The Appeal Rights and a copy of this Licensing report were provided. A notice of site visit was posted and must remain for 30 days. |
