32 | Staff may not have been able to see the injury as C1 was wearing long sleeves, however it should have been observed by removing the sleeve, throughout the day based upon the morning incident. Staff stated they observed the child acting normally after naptime, but did not remove the sleeve to look at the arm.
LPA Lane is also concerned about the cushioning (turf only) under the smaller playground structure (labeled 2-5 years) where the incident on 6/4/24 occurred. LPA advised Director to have it checked for appropriate cushioning and to send LPA any documentation regarding the cushioning. LPAs also observed a second play structure on the other side of the playground that is fenced off from the other two playgrounds. This structure is very tall – over 6 feet in height in some areas which also had a label of 2-5 years of age. This structure is new and hasn’t been used yet. LPA asked Director to not use the playground structure until licensing has cleared it, as it appears this might be an error since the structure does not seem appropriate for the ages 2-5 years. The same type of turf is under this structure which LPA feels does not allow proper cushioning in case of a fall. Director stated she would not use the new structure until Licensing has followed up with the facility.

LPA Keturah Lane informed facility representative director Lori Jiannuzzi that this report dated 6/21/24 documents 1 Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Keturah Lane informed the facility representative to provide a copy of this licensing report dated 6/21/24 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview conducted and report was reviewed with facility representative director Lori Jiannuzzi. A notice of site visit was given and must remain posted for 30 days.
